Hubungan Lama Waktu Operasi terhadap Penurunan Kreatinin 48 Jam setelah Transplantasi Ginjal di Rumah Sakit Cipto Mangunkusumo pada 2019-2024 = Association Between Operative Duration and 48-Hour Post-Transplant Creatinine Reduction at Cipto Mangunkusumo Hospital from 2019 to 2024.

Latar Belakang Penyakit Ginjal Tahap Akhir (PGTA) memiliki tingkat morbiditas dan mortalitas yang tinggi, sehingga pasien PGTA memerlukan Terapi Pengganti Ginjal (TPG). Transplantasi ginjal menjadi tata laksana pilihan yang menawarkan kualitas hidup dan tingkat kelangsungan hidup yang lebih tinggi dibandingkan dialisis. Namun, komplikasi umum seperti Delayed Graft Function (DGF) dapat terjadi pada sekitar 4–50% pasien. Creatinine Reduction Ratio 48 jam (CRR2) merupakan indikator objektif yang efektif untuk menilai cedera ginjal akut dan memprediksi keberhasilan fungsi ginjal jangka panjang setelah transplantasi. Kejadian DGF dapat dipicu oleh faktor risiko seperti usia donor dan resipien, serta lama waktu operasi (Warm Ischemic Time (WIT) dan Cold Ischemic Time (CIT)). Penelitian ini bertujuan untuk mengetahui hubungan lama waktu operasi terhadap penurunan kreatinin 48 jam setelah transplantasi ginjal di RSCM pada periode 2019–2024. Metode Penelitian ini menggunakan desain analitik observasional kohort retrospektif pada 338 resipien transplantasi ginjal donor hidup dengan teknik laparoscopic living donor nephrectomy (LLDN). Analisis bivariat dilakukan menggunakan uji korelasi Spearman dan Mann-Whitney karena seluruh variabel tidak berdistribusi normal (p < 0,05). Hasil Median CRR2 adalah 67,58%. Usia resipien menunjukkan korelasi negatif yang signifikan dengan CRR2 (r = -0,282; p = 0,000). Analisis Mann–Whitney menunjukkan nilai CRR2 yang lebih tinggi pada resipien berusia ≤37 tahun (p = 0,000). Tidak ditemukan korelasi signifikan antara CRR2 dengan usia donor, WIT 1, WIT 2, dan CIT. Kesimpulan Usia resipien merupakan faktor yang berhubungan signifikan dengan penurunan kreatinin 48 jam pasca transplantasi ginjal, sedangkan lama waktu operasi tidak menunjukkan hubungan bermakna.

Introduction End-Stage Renal Disease (ESRD) is associated with high morbidity and mortality, requiring Renal Replacement Therapy (RRT). Kidney transplantation remains the treatment of choice, offering better quality of life and survival rates compared to dialysis. However, common complications such as Delayed Graft Function (DGF) may occur in approximately 4–50% of patients. The 48-hour Creatinine Reduction Ratio (CRR2) is an objective indicator used to assess acute kidney injury and predict long-term graft function after transplantation. DGF can be influenced by several risk factors, including donor and recipient age, as well as intraoperative factors such as Warm Ischemic Time (WIT) and Cold Ischemic Time (CIT). This study aimed to determine the relationship between operative duration and the reduction of creatinine levels within 48 hours after kidney transplantation at Cipto Mangunkusumo General Hospital (RSCM) during the period 2019–2024. Method An analytic observational study with a retrospective cohort design was conducted on 338 living donor kidney transplant recipients who underwent laparoscopic living donor nephrectomy (LLDN). Data were analyzed using the Spearman correlation and Mann– Whitney test due to non-normal variable distribution (p < 0.05). Results The median CRR2 was 67.58%. Recipient age showed a significant negative correlation with CRR2 (r = -0.282; p = 0.000). Mann–Whitney analysis revealed higher CRR2 in recipients aged ≤37 years (p = 0.000). No significant correlation was found between CRR2 and donor age, WIT 1, WIT 2, or CIT. Conclusion Recipient age significantly correlates with 48-hour creatinine reduction, while operative duration shows no significant association.
