Prediktor Rekurensi Penyakit Blount Pasca Koreksi Akut Dengan Osteotomi Tibia Proksimal Dan Fiksasi Internal = Predictors of Recurrance in Blount Disease Following Acute Correction with Proximal Tibial Osteotomy and Internal Fixation.

Latar belakang: Penyakit Blount atau Infantile Tibia Vara (ITV) adalah suatu kondisi gangguan tumbuh kembang yang mengakibatkan deformitas multiplanar pada anggota gerak bawah. Koreksi deformitas Blount dapat dilakukan secara akut namun angka rekurensi setelah tindakan osteotomi yang dilaporkan beberapa peneliti cukup tinggi yaitu 40%-70%. Tujuan dari penelitian ini adalah mengevaluasi faktor-faktor yang kemungkinan berperan sebagai faktor rekurensi pasca koreksi akut penyakit Blount. Metode: Dilakukan studi kohort retrospektif yang membandingkan hubungan usia operasi, status early walker, stadium Langenskiold, sudut TFA, dan sudut MDA terhadap rekurensi pada pasien Blount. Sampel diambil dari pasien usia 4-18 tahun dengan Blount infantil yang menjalani prosedur koreksi akut di RS Cipto Mangunkusumo tahun 2018 – 2022 dengan minimal follow up 1 tahun. Hasil: Total sampel yang digunakan adalah 42 ekstremitas dari 33 pasien. Sebanyak 14 sampel (33.3%) mengalami rekurensi. Dari analisis bivariat didapatkan usia pasien saat operasi lebih dari 4 tahun memiliki risiko yang lebih tinggi untuk mengalami rekurensi (p= 0.031) dan RR (IK95%) = 5.82 (0.84-39.98). Tidak terdapat hubungan signifikan antara status early walker, BMI, stadium Langenskiold, sudut TFA pre dan pasca operasi, sudut MDA pre dan pasca operasi, dengan rekurensi penyakit Blount (p>0.05) Kesimpulan: Pasien Blount yang dilakukan operasi usia diatas 4 tahun memiliki risiko tinggi terjadi rekurensi .

Background: Blount's disease, or infantile tibia vara (ITV), is a developmental disorder that results in multiplanar deformities of the lower extremities. Correction of the deformity can be performed acutely, but recurrence rates after osteotomy, reported by some researhers are quite high, at 40%-70%. The purpose of this study was to evaluate factors that may contribute to recurrence after correction of Blount’s disease . Methods: A retrospective cohort study was conducted comparing the relationship between age at surgery, early walker, Langenskiold stage, TFA angle, and MDA angle with recurrence in Blount's patients. Samples were taken from patients aged 4-18 years with infantile Blount's who underwent acute correction procedures at Cipto Mangunkusumo Hospital between 2018 and 2022 with a minimum follow-up of 1 year. Results: A total of 42 extremities from 33 patients were sampled. Fourteen (33.3%) experienced recurrence. Bivariate analysis showed that patients with an operating age >4 years had a significantly higher risk of experiencing Blount's disease recurrence with p=0.031 and RR (IK95%) = 5.82 (0.84-39.98). There was no significant association between patients early walker status, BMI, Langenskiold stage, pre and post operative TFA angle, pre and post operative MDA angle with Blount's disease recurrence (p>0.05). Conclusion: Blount patients who undergo surgery at the age of over 4 years have a high risk of recurrence
