Perbedaan Variabilitas Denyut Jantung, Depresi, dan Kualitas Hidup Pada Pasien Diabetes Melitus Tipe 2 dan Tanpa Diabetes Melitus Tipe 2 yang Menjalani Hemodialisis Rutin = Differences in Heart Rate Variability, Depression, and Quality of Life in Patients with Type 2 Diabetes Mellitus and Without Type 2 Diabetes Mellitus Undergoing Routine Hemodialysis.

Latar Belakang: Penyakit Ginjal Kronis (PGK) merupakan masalah kesehatan global yang progresif, dengan prevalensi tinggi pada pasien dengan Diabetes Melitus Tipe 2 (DMT2). Salah satu terapi utama PGK stadium akhir adalah hemodialisis (HD). Kualitas hidup pasien PGK yang menjalani HD cenderung rendah akibat penyakit kronis, komplikasi yang menyertainya, serta berbagai stresor yang memengaruhi kondisi fisik dan psikologis. Penelitian ini memiliki kebaruan signifikan melalui pendekatan terintegrasi yang menganalisis aspek fisiologis, psikologis, dan kualitas hidup pada pasien PGK yang menjalani HD rutin. Tujuan: Mengetahui perbedaan VDJ, depresi, dan kualitas hidup antara DMT2 dan tanpa DMT2 pada pasien yang menjalani HD rutin. Metode: Penelitian cross-sectional melibatkan 112 pasien PGK dewasa yang menjalani HD rutin di RS Cipto Mangunkusumo dibagi menjadi dua kelompok, yakni DMT2 dan tanpa DMT2. VDJ diukur sebelum prosedur HD menggunakan alat Photoplethysmograph SA-3000P, dengan fokus pada parameter SDNN dan rasio LF/HF. Depresi dinilai menggunakan kuesioner Hospital Anxiety and Depression Scale – Depression Subscale (HADS-D), sedangkan kualitas hidup dievaluasi dengan instrumen Kidney Disease Quality of Life-36 (KDQOL-36). Perbedaan rerata dievaluasi menggunakan analisis statistik chi-square dan multivariat. Hasil: Terdapat perbedaan signifikan VDJ, SDNN (p < 0,0001) dan rasio LF/HF (p = 0,0004). Tidak terdapat perbedaan signifikan pada depresi (p = 0,409) dan kualitas hidup (p= 0,835). Kesimpulan: Pasien DMT2 menunjukkan derajat disfungsi otonom kardiovaskular yang lebih berat dibandingkan pasien tanpa DMT2. Monitoring HRV secara rutin dapat memberikan nilai prognostik tambahan dalam tata laksana pasien HD, terutama dengan komorbiditas DMT2.

Background: Chronic Kidney Disease (CKD) is a progressive global health problem, with a high prevalence in patients with Type 2 Diabetes Mellitus (T2DM). One of the main therapies for end-stage CKD is hemodialysis (HD). The quality of life of CKD patients undergoing HD tends to be low due to chronic disease, accompanying complications, and various stressors that affect physical and psychological conditions. This study has significant novelty through an integrated approach that analyzes physiological, psychological, and quality of life aspects in CKD patients undergoing regular HD. Objective: To determine the differences in VDJ, depression, and quality of life between DMT2 and non-DMT2 patients undergoing routine HD. Methods : The cross-sectional study involved 112 adult PGK patients undergoing routine HD at Cipto Mangunkusumo Hospital, divided into two groups: DMT2 and non-DMT2. VDJ was measured before the HD procedure using the Photoplethysmograph SA-3000P device, focusing on the SDNN parameter and the LF/HF ratio. Depression was assessed using the Hospital Anxiety and Depression Scale – Depression Subscale (HADS-D) questionnaire, while quality of life was evaluated using the Kidney Disease Quality of Life-36 (KDQOL-36) instrument. Results: There were significant differences in VDJ, SDNN (p < 0.0001) and the LF/HF ratio (p = 0.0004). There were no significant differences in depression (p = 0.409) and quality of life (p = 0.835). Conclusion: Patients with T2DM showed a more severe degree of cardiovascular autonomic dysfunction than patients without T2DM.
