Hubungan antara Karakteristik Sendi Facet Terhadap Derajat Keparahan dan Disabilitas Pasien Spondilolistesis Lumbar Degeneratif = The Relationship Between Facet Joint Characteristics and the Severity and Disability Level in Patients with Degenerative Lumbar Spondylolysthesis.

Pendahuluan: Spondilolistesis lumbar degeneratif merupakan kelainan tulang belakang yang sering dijumpai pada populasi usia lanjut. Karakteristik morfologis sendi facet, seperti orientasi, tropisme, efusi cairan (Facet Fluid Index/FFI), serta rasio tinggi diskus (Farfan Index), diduga berkontribusi terhadap instabilitas segmen lumbar dan tingkat disabilitas pasien. Derajat keparahan secara radiologis seringkali tidak sesuai dengan klinis pasien, klinis ringan pada pasien dapat ditemukan pada derajat listhesis yang berat dan begitu juga sebaliknya. Penelitian ini bertujuan untuk mengevaluasi hubungan antara karakteristik radiologis sendi facet dengan derajat keparahan spondilolistesis berdasarkan klasifikasi Meyerding dan kualitas hidup berdasarkan skor Oswestry Disability Index (ODI) untuk menemukan faktor yang paling memperberat. Metode: Penelitian ini menggunakan desain potong lintang terhadap 100 pasien yang didiagnosis dengan spondilolistesis lumbar degeneratif di Rumah Sakit Cipto Mangunkusumo selama periode 201 9–2024. Evaluasi radiologis dilakukan menggunakan MRI, meliputi pengukuran Tropisme facet, Farfan Index, Facet fluid index, dan orientasi sendi facet. Seluruh parameter dikaitkan secara statistik terhadap derajat keparahan Meyerding dan kategori disabilitas berdasarkan ODI menggunakan uji Chi-square, Mann Whittney, dan Kruskal Wallis. Hasil dan Diskusi: Hanya Farfan Index yang menunjukkan hubungan yang signifikan terhadap kualitas hidup pasien (p < 0,001), di mana nilai Farfan Index yang lebih rendah berhubungan dengan disabilitas yang lebih berat. Tidak ditemukan hubungan bermakna antara parameter lain seperti Tropisme facet, Facet Fluid Index, atau orientasi facet dengan derajat keparahan maupun skor disabilitas pasien. Temuan ini juga menyoroti pentingnya penurunan tinggi diskus sebagai indikator potensial nyeri diskogenik yang berdampak pada kualitas hidup pasien. Kesimpulan: Farfan indeks merupakan parameter radiologis yang paling konsisten dan signifikan berhubungan dengan kualitas hidup dan peningkatan disabilitas, dengan nilai yang rendah memiliki hubungan signifikan dengan penurunan kualitas hidup pasien spondilolistesis degeneratif dan mencerminkan tipe nyeri diskogenik. Hal ini membuka potensi penggunaan Farfan Index sebagai panduan seleksi terapi kontrol nyeri invasif minimal seperti injeksi steroid intradiskal dan epidural.

Introduction: Degenerative lumbar spondylolisthesis is a common spinal disorder, especially in the elderly population. Morphological changes of the facet joints, such as facet orientation, tropism, facet joint effusion (Facet Fluid Index, FFI), and intervertebral disc height (Farfan Index), are believed to play a key role in the pathogenesis and symptom severity of this condition. Radiologically evaluated severity doesn’t seem to always align with the patient’s clinical symptoms. This study aims to evaluate the relationship between facet joint characteristics and both the severity of spondylolisthesis (based on the Meyerding classification) and patient-reported disability (measured by the Oswestry Disability Index, ODI) to find specific radiological factors that could affect the condition the most. Methods: A cross-sectional study was conducted involving 100 patients diagnosed with degenerative lumbar spondylolisthesis at Dr. Cipto Mangunkusumo National General Hospital between 2019 and 2024. Radiological evaluations were performed using MRI and included measurements ofFacet tropism, the Farfan Index, Facet Fluid Index, and facet orientation. These parameters were statistically analyzed for their association with the degree of spondylolisthesis (Meyerding classification) and disability levels (ODI categories) using Chi-square, Mann-Whitney, and Kruskal Wallis. Results and Discussion: Only the Farfan Index showed a statistically significant relationship with patient quality of life (p < 0.001), where lower Farfan Index values were associated with higher levels of disability. Other parameters such as facet tropism, facet fluid index, and facet orientation, did not show significant associations with either the severity ofthe listhesis or the ODI score. This finding highlights the potential role of disc height collapse as an indicator of discogenic pain and reduced functional status in these patients. Conclusion: Farfan index is the most consistent and significantly correlated radiological parameter with quality of life and increase in disability, with a lower Farfan Index significantly associated with worse quality of life in patients with degenerative spondylolisthesis, suggesting a possible discogenic pain mechanism. This supports the potential use of the Farfan Index as a biomechanical parameter to guide minimally invasive pain control strategies, such as intradiscal steroid injections and epidural interventions.
