Perbandingan Quality Of Life Menggunakan Chronic Venous Insufficiency Questionnaire (CIVIQ) Dan Faktor-Faktor Yang Memengaruhi Nya Pada Pasien Chronic Vein Insufficiency Pasca Endovenous Microwave Ablation Dengan Ligasi Tinggi + Stripping = Comparison of Quality of Life Using the Chronic Venous Insufficiency Questionnaire (CIVIQ) and Its Influencing Factors in Patients with Chronic Venous Insufficiency Following Endovenous Microwave Ablation with High Ligation and Stripping.

Latar Belakang. Prevalensi Insufisiensi Vena Kronik (Chronic Venous Insufficiency/CVI) terus meningkat secara global, dengan sekitar 30-40% populasi dewasa mengalami varises sebagai manifestasi awal insufisiensi vena. Hingga saat ini, masih sedikit penelitian yang membandingkan ablasi endovena menggunakan gelombang mikro (Endovenous Microwave Ablation/EMA) dengan tatalaksana konvensional ligasi tinggi dan stripping (High Ligation and Stripping/HLS) dalam hal kualitas hidup pada pasien CVI. Tujuan. Penelitian ini bertujuan untuk menilai perbedaan kualitas hidup pasien CVI pasca tindakan menggunakan kuesioner CIVIQ- 20, dengan membandingkan pasien yang ditatalaksana dengan EMA versus HLS. Metode. Studi kohort prospektif ini melibatkan 40 pasien CVI dari RSCM, RS Hermina Bekasi, dan RS Hermina Depok pada periode Mei-Juni 2025. Data dikumpulkan menggunakan kuesioner. Analisis bivariat dengan Uji Chi-Square, Fischer’s exact test, Independent T-test, dan Mann-Whitney U test. Hasil. Secara keseluruhan, rerata perubahan (Δ) skor CIVIQ untuk EMA vs. HLS adalah 12,82 ± 6,97 vs. 14,33 ± 6,78 (p = 0,334). Pada pasien berusia ≥ 60 tahun, Δ skor CIVIQ untuk HLS vs. EMA adalah 16,17 ± 5,67 vs. 10,89 ± 2,67 (p = 0,029). Pada pasien perempuan, Δ skor CIVIQ untuk HLS vs. EMA adalah 16,63 ± 5,32 vs. 10,23 ± 4,21 (p = 0,006). Tidak ditemukan perbedaan signifikan Δ skor CIVIQ antara HLS dan EMA pada pasien berusia < 60 tahun, laki-laki, atau pasien dengan obesitas dibandingkan dengan normoweight. Simpulan. Pada pasien CVI berusia ≥ 60 tahun dan pasien CVI perempuan, tatalaksana dengan HLS menghasilkan Δ skor CIVIQ-20 yang secara signifikan lebih tinggi dibandingkan EMA. Hal ini menunjukkan kualitas hidup yang lebih baik pada kedua kelompok pasien tersebut ketika ditangani dengan HLS.

Background. The prevalence of Chronic Venous Insufficiency (CVI) continues to increase globally, with approximately 30–40% of the adult population experiencing varicose veins as an early manifestation of venous insufficiency. To date, few studies have compared endovenous microwave ablation (EMA) with conventional high ligation and stripping (HLS) management in terms of quality of life among CVI patients. Objectives. This study aims to assess differences in quality of life among CVI patients post-procedure using the CIVIQ-20 questionnaire, comparing patients managed with EMA versus HLS. Methods. This prospective cohort study involved 40 CVI patients from RSCM, Hermina Hospital Bekasi, and Hermina Hospital Depok during May–June 2025. Data were collected via questionnaire. Bivariate analysis was performed using the Chi-square test, Fisher’s exact test, Independent T-test, and MannWhitney U test. Result. Overall, the mean change (Δ) in CIVIQ scores for EMA vs. HLS was 12.82 ± 6.97 vs. 14.33 ± 6.78 (p = 0.334). Among patients aged ≥ 60 years, the Δ CIVIQ score for HLS vs. EMA was 16.17 ± 5.67 vs. 10.89 ± 2.67 (p = 0.029). Among female patients, the Δ CIVIQ score for HLS vs. EMA was 16.63 ± 5.32 vs. 10.23 ± 4.21 (p = 0.006). No significant difference in Δ CIVIQ scores between HLS and EMA was found in patients aged < 60 years, male patients, or obese patients compared to those with normal weight. Conclusion. For CVI patients aged ≥ 60 years and female CVI patients, management with HLS resulted in a significantly higher Δ CIVIQ-20 score compared to EMA. This indicates better quality of life in both patient groups when managed with HLS.
