Hubungan antara Visceral Adiposity Index (VAI) dengan Penurunan Laju Filtrasi Glomerulus (LFG) = Association between Visceral adiposity index (VAI) and Declining Estimated Glomerulus Filtration Rate (eGFR).

Latar belakang: Obesitas berhubungan dengan terjadinya PGK terutama berhubungan dengan peningkatan lemak viseral. VAI diketahui merupakan salah satu parameter yang dapat memprediksi penumpukan lemak viseral. Hingga saat ini belum ada penelitian mengenai hubungan VAI dengan penurunan LFG dan insidensi PGK di Indonesia. Tujuan: Mengetahui korelasi VAI dengan penurunan LFG dan hubungan VAI dengan insidensi PGK. Metode: Penelitian dengan desain kohort retrospektif pada data Penelitian Kohort Penyakit Tidak Menular (PTM) Litbangkes Republik Indonesia Tahun 2011-2020. Subjek kemudian dikelompokan menjadi PGK dan non PGK pada awal penelitian dan dilakukan analisis lebih lanjut pada kelompok non PGK. Analisis dengan Uji korelasi Spearman, uji bivariat dengan chi-square dan multivariat dengan regresi logistik. Hasil: Didapatkan subjek sebanyak 976 dan terbagi menjadi 969 subjek non PGK dan 7 subjek PGK yang diikutkan dalam analisis. Ditemukan bahwa subjek dengan PGK diawal memiliki profil rerata TDS, TDD, IMT dan VAI lebih tinggi dibandingkan non PGK. Berdasarkan uji korelasi ditemukan bahwa korelasi positif sangat lemah antara VAI dan penurunan LFG dengan r=0.011 dan tidak bermakna secara statistik dengan p=0.74. Analisis tambahan dilakukan uji bivariat dan multivariat menunjukan bahwa VAI berhubungan dengan insidensi PGK secara independent dengan nilai OR (IK95%) 1.92 (1.31-2.82) dan bermakna secara statistik dengan nilai p=0.001. Variabel lain yang berhubungan secara independent adalah usia dan jenis kelamin. Kesimpulan: VAI tidak berkorelasi dengan penurunan LFG. Performa VAI dengan penurunan LFG pada kelompok PGK dan Non PGK tidak dapat dilanjutkan.

Background: Obesity is associated with the occurrence of CKD, primarily related to an increase in visceral fat. VAI is known to be one of the parameters that can predict visceral fat accumulation. To date, no studies have been conducted on the relationship between VAI and the decline in GFR and the incidence of CKD in Indonesia. Objective: To investigate the correlation between VAI and declining eGFR, as well as the incidence of CKD. Methods: A retrospective cohort study was conducted using data from the Indonesian Research and Development Cohort of Non-Communicable Diseases (2011–2020). Participants were categorized into CKD and non-CKD groups. The relationship between VAI and declining eGFR was analyzed using the Spearman correlation test. Bivariate analysis was conducted using the chi-square test, followed by multivariate analysis with logistic regression to determine independent associations. Results: At baseline in 2011, 976 participants were included in the study, with 969 in the non-CKD group and 7 in the CKD group. By 2020, the number of CKD cases had increased to 159. Correlation analysis revealed a very weak positive correlation between VAI and declining eGFR (r = 0.011), which was not statistically significant (p = 0.74). However, bivariate and multivariate analyses demonstrated that VAI was independently associated with CKD incidence, with an odds ratio (OR) of 1.92 (95% CI: 1.31–2.82; p = 0.001). Other variables independently associated with CKD included age and gender. Conclusion: VAI does not correlate with a decrease in LFG. The performance of VAI with a decrease in LFG in CKD and Non-CKD groups cannot be continued.
