Faktor-faktor yang memengaruhi kebocoran bilier pada pasien anak pascatransplantasi hati donor hidup di RSUPN dr Cipto Mangunkusumo = Factors affecting biliary leakage in pediatric patients after liver transplantation from living donors at RSUPN dr. Cipto Mangunkusumo.

Latar belakang: Transplantasi hati anak yang dilakukan di RSCM sejak tahun 2010 hingga November 2024 sudah dilakukan 84 resipien dengan keluaran komplikasi yang berbeda-beda. Terjadinya komplikasi pada resipien ini meningkatkan angka morbiditas dan mortalitas sehingga Penelitian ini dilakukan untuk mengetahui faktor-faktor yang memengaruhi kebocoran bilier pada pasien anak pascatransplantasi hati donor hidup. Metode: Penelitian ini menggunakan kohort retrospektif melibatkan seluruh resipien anak pascatransplantasi donor hidup di RSCM. Berdasarkan data rekam medis, setiap pasien dinilai jumlah sambungan bilier, warm ischemic time, cold ischemic time, lama operasi, jumlah perdarahan intraoperasi, dan komplikasi arteri hepatika. Faktor-faktor tersebut kemudian dianalisis hubungan dengan kejadian kebocoran bilier. Hasil: Terdapat delapan puluh resipien anak pascatransplantasi donor hidup di RSCM dengan median usia subjek 17 bulan (6-165 bulan). Dari delapan puluh subjek, tujuh pasien dengan kebocoran bilier dianalisis tidak didapatkan faktor-faktor yang bermakna secara statistik dengan kejadian kebocoran bilier. Konklusi: Faktor-faktor yang dianalisis tidak menunjukkan hubungan dengan kejadian kebocoran bilier pada resipien anak pascatransplantasi donor hidup di RSCM. Akan tetapi perlu diperhatikan faktor lain seperti jumlah sampel yang dapat memengaruhi hasil dan masih perlu ditingkatkan pada studi ini.

Introduction: Liver transplantation in pediatric performed at RSCM from 2010 to November 2024 has been performed on 84 recipients with different complication outcomes. The occurrence of complications in these recipients increases morbidity and mortality rates, so this study was conducted to determine the factors that influence biliary leakage in pediatric patients after living donor liver transplantation. Methods: his study used a retrospective cohort involving all pediatric recipients of living donor transplants at RSCM. Based on medical records, each patient was evaluated for the number of biliary anastomoses, warm ischemic time, cold ischemic time, duration of surgery, amount of intraoperative bleeding, and hepatic artery complications. These factors were then analyzed for their correlation with the occurrence of biliary leakage. Results: There were eighty pediatric recipients of living donor transplants at RSCM with a median age of 17 months (6-165 months). Eighty subjects, seven patients with biliary leakage, were analyzed, and no statistically significant factors were found to be associated with the occurrence of biliary leakage. Conclusion: The analyzed factors did not show a correlation with the occurrence of biliary leakage in pediatric recipients after living donor transplantation at RSCM. However, other factors such as the sample size, which may influence the results, should be considered and need to be improved in this study.
