Profil Lipoprotein Aterogenik dan Hubungannya dengan Resistensi Insulin pada First Degree Relatives Diabetes Melitus Tipe 2 = Atherogenic Lipoprotein Profile and Its Correlation with Insuline Resistance in First Degree Relatives Type 2 Diabetes Mellitus.

Latar belakang: Individu dengan keluarga derajat pertama penyandang diabetes melitus tipe 2 atau yang disebut dengan first degree relative (FDR) DM tipe 2 dihadapkan pada berbagai gangguan metabolik yang terjadi lebih dini terutama resistensi insulin dan gangguan metabolisme lemak yang membuat kelompok ini berisiko mengidap penyakit kardiovaskular lebih besar dibandingkan populasi lainnya. Belum ada penelitian di Indonesia yang mencari korelasi antara lipoprotein aterogenik pada populasi FDR usia muda yang masih normotensi dan normoglikemi. Tujuan: Mengetahui beda rerata kadar sdLDL dan rasio apoB/apoA-I antara FDR dan non-FDR DM tipe 2 serta mengetahui korelasi antara kadar sdLDL dan rasio apoB/apoAI dengan HOMA-IR pada FDR DM tipe 2. Metode: Desain studi potong lintang, data diambil dari penelitian payung pada Divisi Metabolik Endokrin dan Diabetes Departemen Ilmu Penyakit Dalam FKUI/ RSCM yang berjudul “Gangguan Kardio-Vaskular-Metabolik Dini pada Populasi First Degree Relative Diabetes Mellitus tipe 2”. Pemeriksaan sdLDL, apoB, apoA-I kemudian ditambahkan pada sampel studi sebelumnya. Analisis bivariat dan uji korelasi dilakukan terhadap variabel sdLDL, rasio apoB/apoA-I, dan HOMA-IR. Hasil: Dari 125 subjek penelitian yang terdiri dari 62 subjek FDR dan 63 subjek Non FDR, ditemukan perbedaan bermakna antara kadar sdLDL pada kelompok FDR dan non FDR (31,42 (20,1-41,39) vs 21,05 (12,18-26,13) mg/dL, p < 0,0001 ). Perbedaan yang bermakna juga didapatkan pada rasio apoB/apoA-I pada kelompok non FDR dan non FDR (1,97 (1,36-2,6) vs 2,42 (1,95-3,16), p=0,002). Tidak ditemukan korelasi bermakna antara kadar sdLDL dengan HOMA-IR pada kelompok FDR (r=0,059, p=0,649), namun ditemukan korelasi positif sedang yang bermakna pada rasio apoB/apoA-I (r=0,475, p < 0,0001). Kesimpulan. Didapatkan perbedaan rerata sdLDL dan rasio apoB/apoA-I antara kelompok FDR dan Non FDR. Rasio apoB/apoA-I berkorelasi positif terhadap HOMAIR pada kelompok FDR.

Background: First-degree relative (FDR) of insividuals with type 2 diabetes (T2DM) are at higher risk os developing early metabolic disturbances, particularly insulin resistance and lipid metabolism abnormalities. These issues contribute to a greater predisposition to cardiovascular disease compared to the general populations. However, research on the relationship between atherogenic lipoproteins of normotensive, normoglycemic young FDRs in Indonesia remains limited. Objective: To evaluate the association between small, dense low density lipoprotein (sdLDL) levels and HOMA-IR in FDRs T2DM, and to assess differences sdLDL levels between FDR and non-FDR group. Methods: This cross-sectional study analyzed secondary data from the Metabolic Endocrine and Diabetes Division of Internal Medicine Department of Faculty Medicine of Universitas Indonesia and Cipto Mangunkusumo Hospital. The primary study titled “Gangguan Kardio-Vaskular-Metabolik Dini pada Populasi First Degree Relative Diabetes Mellitus tipe 2” was expanded to include sdLDL measurements. Bivariate analysis and correlation test were used to explore the relationship between sdLDL and HOMA-IR. Result: Among 125 subjects consisting of 62 FDR and 63 non-FDR individuals, sdLDL levels were significantly higher in the FDR group compared to the non-FDR group (31.42 (20.1-41.39) vs 21.05 (12, 18-26.13) mg/dL, p < 0.0001). However, no significant correlation was observed between sdLDL levels and HOMA-IR in the FDR group (r=0.059, p=0.649). Conclusion: There was a significant difference in sdLDL levels between FDRs and nonFDRs of T2DM patients, but no correlation was found between sdLDL and HOMA-IR in the FDR group.
