Fiksasi Rekonstruksi Ligamentum Krusiatum Posterior Menggunakan Teknik Transtibial Dibandingkan Tibial Inlay: Sebuah Tinjauan Sistematis = Posterior Crucial Ligamentum Reconstruction Fixation Using Transtibial Techniques Versed To Tibial Inlay: A Systematic Review.

Pendahuluan: PCL merupakan ligament terbesar pada lutut manusia yang berfungsi menahan translasi tibialis posterior pada semua gerakan fleksi lutut. Cedera berat pada PCL menjadi salah satu indikasi dilakukannya rekonstruksi PCL. Dalam rekonstruksi PCL, prosedur fiksasi graft pada tibia dapat dilakukan dengan teknik Transtibial atau Tibial inlay. Kedua teknik tersebut dianggap memiliki kelebihan dan kekurangannya sendiri. Teknik yang memberikan keberhasilan yang lebih baik saat ini masih belum diketahui. Metode: Pencarian literatur mendalam dilakukan pada database Pubmed, Scopus, Cochrane, dan ScienceDirect. Skrining literatur dilakukan menggunakan kriteria inklusi dan eksklusi sesuai dengan panduan PRISMA. Artikel yang lolos seleksi diuji eligibilitasnya dengan Newcastle Ottawa Scale untuk penelitian kohort non-randomized. Hasil: Sebanyak 6 dari 49 literatur yang ditemukan menggunakan kata kunci diikutkan dalam penelitian ini. Seluruh studi merupakan kohort retrospektif non random yang mencakup total 312 pasien. Rerata masa follow up adalah 25 sampai 198 bulan. Tidak ditemukan perbedaan angka re-rupture dari kedua teknik. Luaran fungsional yang diukur dalam Lysholm Knee Score dan Tegner Activity Scale menunjukkan perbaikan signifikan dari preoperasi dengan postoperasi. Perbandingan kedua teknik pada seluruh literatur menunjukkan perbedaan yang tidak signifikan. Kesimpulan: Tidak ditemukan perbedaan tingkat re-rupture ataupun luaran fungsional yang bermakna dari dilakukannya teknik transtibial ataupun tibial inlay. Kedua teknik memberikan perbaikan luaran klinis postoperasi yang bermakna.

Introduction: The PCL is the largest ligament in the human knee that functions to resist posterior tibial translation in all knee flexion movements. Severe injury to the PCL is one of the indications for PCL reconstruction. In PCL reconstruction, the graft fixation procedure on the tibia can be performed with Transtibial or Tibial inlay techniques. Both techniques are considered to have their own advantages and disadvantages. The technique that provides better success is currently unknown. Methods: An in-depth literature search was conducted on Pubmed, Scopus, Cochrane, and ScienceDirect databases. Literature screening was performed using inclusion and exclusion criteria according to PRISMA guidelines. Selected articles were tested for eligibility using the Newcastle Ottawa Scale for non-randomized cohort studies. Results: A total of 6 out of 49 literatures found using keywords were included in this study. All studies were non-randomized retrospective cohorts that included a total of 312 patients. The mean follow-up period was 25 to 198 months. There was no difference in re-rupture rate in both groups. Clinical outcomes measured in Lysholm Knee Score and Tegner Activity Scale showed significant improvement from preoperative to postoperative. Comparison of the two techniques across the literature showed nonsignificant differences. Conclusion: There was no significant difference in re-rupture rate or clinical outcomes from performing the transtibial or tibial inlay technique. Both techniques provided significant improvement in postoperative clinical outcomes.
