Evaluasi Hasil Fungsional dan Perubahan Radiologis Pasca Perbaikan Meniskus dan Meniskektomi per Artroskopi: Studi Retrospektif di RSUPN Dr. Cipto Mangunkusumo Periode 2014-2023 = Functional Outcome and Radiological Changes Post Meniscus Repair and Arthroscopic Meniscectomy: A Retrospective Study at RSUPN Dr. Cipto Mangunkusumo Period 2014-2023.

Pendahuluan: Operasi untuk meniscus tear adalah salah satu operasi ortopedi yang paling sering dilakukan. Kondisi ini dapat dioperasi dengan meniscectomy atau meniscus repair. Metode: Sebuah studi kohort retrospektif dilakukan untuk membandingkan perubahan radiologi x-ray dan luaran fungsional pada pasien meniscus tear pasca tindakan meniscus repair dan meniscectomy per artroskopi di Rumah Sakit dr. Cipto Mangunkusumo (RSCM) tahun 2014 - 2023. Radiologi dinilai dengan melihat perubahan derajat osteoartritis pasca operasi dan luaran fungsional berdasarkan yaitu skor International Knee Documentation Committee (IKDC), skala Lysholm knee scale, dan skor Tegner. Hasil: Sebanyak 68 subjek yang diikutsertakan dalam analisis pada penelitian ini dengan 57 subjek menjalani meniscus repair dan 11 subjek menjalani meniscectomy. Pada penelitian ini didapatkan perbaikan fungsional skor lysholm (25 vs 16; p: 0,016); dan skor tegner (3 vs 2; p: 0,031) kelompok meniscus repair lebih superior dibandingkan kelompok meniscectomy. Tidak terdapat perbedaan bermakna perubahan skor IKDC antar kelompok (p: 0,162). Pada kelompok dengan meniscus repair tidak ditemukan adanya perubahan radiologis pasca tindakan. Namun, dari 11 subjek yang menjalani prosedur meniscectomy didapati bahwa terdapat 3 subjek (27,27%) yang mengalami perburukan (p: 0,003). Kesimpulan: Kelompok meniscus repair memiliki luaran fungsional yang lebih tinggi dan progresivitas osteoarthritis lebih lambat apabila dibandingkan dengan kelompok meniscectomy.

Introduction: Surgery for meniscus tears is one of the most frequently performed orthopedic procedures. This condition can be treated with meniscectomy or meniscus repair. Methods: A retrospective cohort study was conducted to compare radiological changes on x-ray and functional outcomes in patients with meniscus tears who underwent arthroscopic meniscus repair and meniscectomy at Dr. Cipto Mangunkusumo Hospital (RSCM) from 2014 to 2023. Radiological assessment was performed by observing changes in the degree of osteoarthritis post-operation, while functional outcomes were evaluated using the International Knee Documentation Committee (IKDC) score, the Lysholm knee scale, and the Tegner score. Results: A total of 68 subjects were included in the analysis, with 57 subjects undergoing meniscus repair and 11 subjects undergoing meniscectomy. The study found that the functional improvement in Lysholm scores (25 vs 1 6; p: 0.016) and Tegner scores (3 vs 2; p: 0.031) was significantly superior in the meniscus repair group compared to the meniscectomy group. There was no significant difference in the change of IKDC scores between the groups (p: 0.162). The meniscus repair group did not show any posttreatment radiological changes. However, among the 11 subjects who underwent meniscectomy, 3 subjects (27.27%) experienced worsening (p: 0.003). Conclusion: The meniscus repair group demonstrated higher functional outcomes and slower progression of osteoarthritis compared to the meniscectomy group.
