Precocious Pseudo-Puberty with Testicular Enlargement: Two Cases of Leydig Cell Tumor with Different Histopathological Results = Pubertas Prekoks Semu dengan Pembesaran Testis: Dua Kasus Tumor Sel Leydig dengan Hasil Histopatologi Berbeda.

Introduction: Leydig cell tumors (LCT) are the most common hormone-secreting testicular tumors; it is a rare cause for precocious pseudo-puberty in boys. The tumors secrete high amounts of testosterone. We present two cases of LCT in prepubertal boys presenting with precocious puberty. Case Report: Case 1. A 6-year-old boy was referred from the pediatric department with a diagnosis of precocious puberty. The patient had reported enlarged and painless swelling of the left testicle from a year ago. The puberty status of the patient was A1P3G4. Ultrasonography examination had found left testicular mass. Elective radical orchiectomy of the left testicle was performed. Histopathological analysis confirmed the diagnosis of benign LCT. Case 2. A 6-yearold boy presented with an enlarged left testicle for the last three months. Features of puberty were noted on the patient (appearance of pubic and facial hair). The puberty status of the patient was A1P3G3. Left testicle US had found homogenous, hypoechoic mass with calcification. Bone age had found increased bone maturation. Increased androgen hormones were detected through a blood test. Radical orchiectomy of the left testicle was performed. The histopathological examination showed malignant LCT. Conclusion: Leydig cell tumors uncommonly occur in children. Prepubertal-aged boys presented with asymmetrical, firm, painless testicular enlargement with signs of puberty should be evaluated for LCT. Histopathological analysis is the mainstay of diagnosis and radical orchiectomy is the treatment of choice of LCT.

Objektif: Tumor sel Leydig (Leydig cell tumors/LCT)adalah tumor testis yang paling umum menghasilkan hormon; ini adalah penyebab langka pseudo-pubertas prekoks pada anak laki-laki. Tumor ini mengeluarkan sejumlah besar testosteron. Kami menyajikan dua kasus LCT pada anak laki-laki prapubertas yang mengalami pubertas dini. Laporan Kasus: Kasus 1. Seorang anak laki-laki berusia 6 tahun dirujuk dari departemen pediatrik dengan diagnosis pubertas prekoks. Pasien melaporkan pembengkakan yang membesar dan tidak nyeri pada testis kiri sejak setahun yang lalu. Status pubertas pasien adalah A1P3G4. Pemeriksaan ultrasonografi terdapat massa testis kiri. Dilakukan orkiektomi radikal elektif pada testis kiri. Analisis histopatologi mengkonfirmasi diagnosis LCT jinak. Kasus 2. Seorang anak laki-laki berusia 6 tahun datang dengan testis kiri yang membesar selama tiga bulan terakhir. Tanda-tanda pubertas terlihat pada pasien (munculnya rambut kemaluan dan wajah). Status pubertas pasien adalah A1P3G3. Ultrasonografi testis kiri menemukan massa homogen, hipoekoik dengan kalsifikasi. Usia tulang menunjukkan peningkatan maturasi tulang. Hormon androgen yang meningkat terdeteksi melalui tes darah. Dilakukan orkiektomi radikal pada testis kiri. Analisis histopatologi mengkonfirmasi diagnosis LCT ganas. Kesimpulan: Tumor sel Leydig jarang terjadi pada anak-anak. Anak laki-laki prapubertas yang menunjukkan pembesaran testis asimetris, keras, dan tidak nyeri dengan tanda-tanda pubertas harus dievaluasi untuk LCT. Analisis histopatologi adalah metode utama untuk diagnosis, dan orkiektomi radikal adalah pilihan pengobatan untuk LCT.
