Efektifitas Analgesia Akupunktur Tanam Benang di Titik EX-B2 Setinggi Lumbal Pertama Ditambah Terapi Standar Terhadap Nyeri Akut dan Kualitas Hidup Pasca Laparoskopi Donor Nefrektomi Hidup = Effectiveness ofThread Embedding Acupuncture Analgesia at the EX B2 Point at the First Lumbar Level Combined With Standard Therapy on Acute Pain and Quality of Life After Laparoscopic Living Donor Nephrectomy.

Pendahuluan: Transplantasi ginjal dari donor hidup merupakan pilihan pengobatan utama bagi penderita penyakit ginjal stadium akhir. Laparoskopi Donor Nefrektomi Hidup (LDNH) merupakan metode operasi terbaik untuk mendapatkan organ dari pendonor. LDNH lebih unggul dibanding operasi bedah terbuka dalam berbagai hal seperti nyeri pasca operasi yang lebih ringan dan perdarahan intra operasi yang lebih sedikit. Namun nyeri masih merupakan komplikasi utama pasca operasi LDNH. Nyeri yang tidak diatasi dapat menyebabkan terjadinya nyeri kronis dan penurunan kualitas hidup pasien. Area nyeri pasca operasi nefrektomi paling sering adalah antara iga dengan pinggul. Akupunktur tanam benang (ATB) merupakan metode akupunktur yang aman dan efektif dalam mengatasi nyeri di daerah punggung bawah. Metode: Desain studi ini adalah serial kasus dengan jumlah sampel 6 pasien donor ginjal. Studi dilakukan dari September 2023 sampai Januari 2024. Penjaruman ATB dilakukan satu kali pada titik EX-B2 setinggi lumbal pertama pada saat 7 hari pre operasi LDNH. Luaran yang dinilai adalah skor nyeri menggunakan kuesioner Visual Analog Scale (VAS) dan kulitas hidup dengan kuesioner Short Form-36 (SF-36). Data penggunaan rescue medicine juga dicatat pada studi ini. Hasil: Persentase pasien pada hari ke-1 pasca operasi yang mengalami nyeri ringan adalah 50% (3 pasien ), nyeri sedang adalah 33,3% (2 pasien) dan nyeri berat adalah 16,6% (1 pasien). Pada hari ke-14 pasca operasi, kualitas hidup 2 pasien (33,3% dari populasi) telah kembali ke kondisi sebelum operasi. Tidak terdapat efek samping pasca penjaruman ATB. Pasien yang menggunakan rescue medicine adalah 3 pasien (50% dari populasi). Kesimpulan: Kombinasi ATB dengan analgetik standar dapat beruguna sebagai pilihan terapi dalam tatalaksana nyeri pasca operasi laparoskopi donor nefrektomi hidup (LDNH).

Introduction: Kidney transplantation from a living donor is the main treatment option for people with end-stage kidney disease. Laparoscopic Living Donor Nephrectomy (LLDN) is the best surgical method for obtaining organs from donors. LLDN is superior to open surgery in various ways such as less postoperative pain and less intraoperative bleeding. However, pain is still the main complication after LLDN surgery. Untreated pain can cause chronic pain and reduce the patient's quality of life. The most common area of pain after nephrectomy surgery is between the ribs and the hips. Thread embedding acupuncture (TEA) is a safe and effective acupuncture method for treating pain in the lower back area. Methods:The design of this study was a case series with a sample size of 6 kidney donor patients. The study was conducted from September 2023 to January 2024. TEA needling was performed once at the EX-B2 point at the level of the first lumbar 7 days before LLDN surgery. The outcomes assessed were pain scores using the Visual Analog Scale (VAS) questionnaire and quality of life using the Short Form-36 (SF-36) questionnaire. Data on the use of rescue medicine was also recorded in this study. Results:The percentage of patients experiencing mild pain on 1st postoperative day was 50% (3 patients), moderate pain was 33.3% (2 patients) and severe pain was 16.6% (1 patient). On the 14th postoperative day, the quality of life of 2 patients (33.3% of the population) had returned to preoperative conditions. There are no side effects after TEA needling. There were 3 patients who used rescue medicine (50% of the population). Conclusion: The combination of TEA with standard analgesics can be useful as a therapeutic option in the management of pain after laparoscopic living donor nephrectomy (LLDN).
