Elektroakupunktur Telinga Battlefield Acupuncture ( BFA ) Dengan Terapi Standar Sebagai Pilihan Terapi Pada Nyeri Akut dan Kualitas Hidup Pasien Pasca Operasi Percutaneous Nephrolithotomy ( PCNL ) = Ear Electroacupuncture on Battlefield Acupuncture ( BFA )with Standard Therapy as a Therapy Option for Acute Pain and Quality of Life in Post Percutaneous Nephrolithotomy ( PCNL ).

Pendahuluan : Percutaneous nephrolithotomy (PCNL) adalah pilihan utama untuk batu ginjal yang berukuran lebih dari 2 cm. Tindakan ini dapat menimbulkan nyeri pasca operasi yang merupakan masalah yang sering terjadi dan dapat mempengaruhi kualitas hidup pasien. Prevalensi nyeri pasca PCNL di Indonesia bervariasi. Penanganan nyeri pasca operasi bertujuan untuk mengurangi atau menghilangkan nyeri dengan efek samping yang minimal. Saat ini, metode standar dalam menangani nyeri pasca operasi yang digunakan di seluruh dunia adalah dengan penggunaan opiod. Namun penggunaan opioid memiliki banyak efek samping dan dapat mempengarui kualitas hidup pada pasien. Sehingga diperlukan tatalaksana yang aman, nyaman dan efektif dalam mengatasi nyeri pasca PCNL, salah satunya adalah dengan Elektroakupunktur telinga Battlefield Acupuncture (BFA). Metode : Desain studi ini adalah serial kasus dengan jumlah sampel 8 pasien PCNL. Studi dilakukan dari November 2023 sampai Januari 2024. Elektroakupunktur telinga BFA dilakukan selama 30 menit pada kedua telinga, satu jam sebelum PCNL. Luaran yang dinilai adalah skor nyeri ( VAS ), kualitas hidup dengan kuesioner Short Form-36 (SF-36) ,penggunaan analgesik juga efek samping yang dialami pasien dicatat pada studi ini Hasil : Terapi elektroakupunktur telinga BFA dapat menurunkan skala nyeri berupa Visual Analog Scale ( VAS ) pada pasien operasi PCNL batu ginjal. Pada 24 jam pasca PCNL dan EA BFA, 7 dari 8 pasien dengan presentase 87,5% pasien mengalami penurunan skor VAS dan pada 7 hari pasca PCNL dan EA BFA, ke 8 pasien dengan presentase 100 % pasien mengalami penurunan skor VAS. Terapi elektroakupunktur telinga BFA juga dapat meningkatkan kualitas hidup pada 7 hari pasca tindakan yang diukur dengan menggunakan short form 36 ( SF36 ) pada pasien pasca PCNL dan EA BFA. Terapi elektroakupunktur telinga BFA aman, tidak menimbulkan efek samping dan pada pasien hanya mendapatkan tambahan terapi Paracetamol 1000mg . Kesimpulan : Terapi Elektroakupunktur BFA dapat diberikan pada pasien PCNL dengan keamanan yang terbukti baik pada ke 8 pasien dengan presentase 100 % pasien tidak mengalami efek samping pasca EA BFA.

Introduction : Percutaneous nephrolithotomy (PCNL) is the main choice for kidney stones larger than 2 cm. This procedure can cause post-operative pain, which is a problem that often occurs and can affect the patient's quality of life. The prevalence of post-PCNL pain in Indonesia varies. Postoperative pain management aims to reduce or eliminate pain with minimal side effects. Currently, the standard method of treating postoperative pain used throughout the world is the use of opioids. However, the use of opioids has many side effects and can affect the patient's quality of life. So safe, comfortable and effective treatment is needed to treat post-PCNL pain, one of which is Battlefield Acupuncture (BFA) ear electroacupuncture. Methods : The design of this study was a case series with a sample size of 8 PCNL patients. The study was conducted from November 2023 to January 2024. BFA ear electroacupuncture was performed for 30 minutes on both ears, one hour before PCNL. The outcomes assessed were pain scores (VAS), quality of life with the Short Form-36 (SF-36) questionnaire, use of analgesics as well as side effects experienced by patients recorded in this study. Results : BFA ear electroacupuncture therapy can reduce the pain scale in the form of a Visual Analog Scale (VAS) in kidney stone PCNL surgery patients. At 24 hours after PCNL and EA BFA, 7 of 8 patients with a percentage of 87.5% of patients experienced a decrease in VAS scores and at 7 days after PCNL and EA BFA, all 8 patients with a percentage of 100% of patients experienced a decrease in VAS scores. BFA ear electroacupuncture therapy can also improve quality of life 7 days after the procedure as measured using the short form 36 (SF36) in patients after PCNL and EA BFA. BFA ear electroacupuncture therapy is safe, does not cause side effects and patients only receive additional 1000mg Paracetamol therapy. Conclusion : BFA Electroacupuncture therapy can be given to PCNL patients with proven safety in 8 patients with a 100% percentage of patients not experiencing side effects after EA BFA.
