Efektivitas Kombinasi Drospirenon dan Etinil Estradiol pada Terapi Akne Vulgaris Derajat Sedang dan Berat dengan Gejala Hiperandrogen = The Efficacy of Drospirenone-Etinyl Estradiol For Treatment of ModerateSevere Acne Vulgaris with Hiperandrogen symptome.

Latar Belakang: Akne vulgaris merupakan masalah kesehatan yang dialami 85% hidup seseorang di Indonesia. Prevalensi terbanyak adalah perempuan, yang berupaya untuk mengobati akne vulgaris. Terapi yang tidak berhasil, dihubungkan dengan pengaruh hormonal, yaitu perangsangan kelenjar sebasea oleh hormon androgen. Hiperandrogenisme dialami oleh 10% wanita di Indonesia. Terapi utama hiperandrogen yaitu ciproteron asetat. Drospirenon yang dikombinasikan dengan etinil estradiol, selain untuk menjarangkan kehamilan, juga diduga efektif mengobati akne vulgaris dan gejala hiperandrogenisme. Saat ini belum ada penelitian tentang drospirenon untuk terapi akne dan hiperandrogen di Indonesia. Tujuan: Mengetahui efektifitas kombinasi drospirenon 3 mg dan etinil estradiol 30 mikrogram dibandingkan dengan ciproteron asetat 2 mg dikombinasikan etinil estradiol 35 mikrogram, pada terapi akne vulgaris derajat sedang-berat dan gejala hiperandrogen (hirsutisme, amenore sekunder dan oligomenore) selama 3 bulan. Metode: Penelitian ini merupakan uji klinik dengan randomisasi dengan penyamaran ganda. Pengambilan sample secara konsekutif berdasarkan tabel alokasi random. Sebanyak 42 subjek didiagnosa akne vulgaris derajat sedang-berat disertai hirsutisme, amenore sekunder dan atau olimenore di RSCM, terbagi menjadi 2 kelompok 21 subjek terapi drospirenon 3 mg kombinasi etinil estradiol 0,030 mg dan 21 subjek merupakan kelompok kontrol terapi ciproteron asetat 2 mg dan etinil estradiol 0,035 mg, diperiksakan testosteron bebas. Terdapat 1 subjek drop out pada kelompok ciproteron asetat. Terapi selama 3 bulan, lalu dievaluasi lesi akne, skor Ferriman Gallwey (skor FG), dan pola haid. Hasil: Karakteristik subjek penelitian adalah sebagian besar usia subjek 25-35 tahun yaitu 51,3% untuk kelompok drospirenon. Tingkat pendidikan subjek terbanyak 51,7% adalah sarjana. Kadar testosteron pada awal penelitian adalah 1,41±0,64 pada kelompok drospirenon dan 1,32±0,48 pada kelompok ciproteron asetat. Pada kelompok drospirenon, untuk derajat akne, 85,7% subjek kelompok drospirenon dan 85% subjek kelompok ciproteron asetat berkurang lesi aknenya dengan nilai P= 0,645. Untuk hirsutisme dengan penghitungan skor FG, sebanyak 61,9% subjek kelompok drospirenon dan 50% subjek kelompok ciproteron asetat dengan skor FG yang menurun dengan nilai P=0,443, dan pola haidnya menjadi teratur sebanyak 85,7% subjek kelompok drospirenon dan 90% subjek kelompok ciproteron asetat dengan P=0,645. Kesimpulan: Drospirenon dan etinil estradiol sama efektifnya dengan ciproteron asetat yang dikombinasikan dengan etnil estradiol pada terapi akne vulgaris derajat sedang-berat, hirsutisme, amenore sekunder dan oligomenore.

Background: Acne vulgaris is a health problem experienced by 85% of people in Indonesia. The highest prevalence is women, who attempt to treat acne vulgaris. Unsuccessful acne vulgaris therapy is associated with hormonal influences, the stimulation of the sebaceous glands by androgen hormones. Hyperandrogenism is experienced by 10% of women in Indonesia. The main therapy for hyperandrogens is ciproterone acetate. Drospirenone combined with ethinyl estradiol, apart from sparing pregnancy, is also thought to be effective in treating clinical symptoms of hyperandrogenism. Currently there is no research on drospirenone for acne and hyperandrogen therapy in Indonesia. Objective: To determine the efficacy of the combination of3-mg drospirenone and 20-microgram ethinyl estradiol in the treatment of moderate-severe acne vulgaris and symptoms of hyperandrogenism (hirsutism, secondary amenorrhea and oligomenorrhea) for 3 cycles. Method: This research is a randomized clinical trial with double blind. Consecutive sampling is based on a random allocation table. A total of 42 subjects were diagnosed with moderate-severe acne vulgaris with hirsutism, secondary amenorrhea and/or olimenorrhea at RSCM, divided into 2 groups, 21 subjects using drospirenone 3 mg combined with 0.030 mg ethinyl estradiol (DRSP/EE) and 21 subject using 2 mg ciproterone acetate and 0.035 mg ethinyl estradiol (CRPN/EE), continued with free testosterone check in laboratory. There was 1 subject who dropped out (Cyproterone group). The duration of therapy was 3 months then evaluation of acne lesions, Ferriman Gallwey score (FG score), and menstrual period. Results: Characteristics of research subjects with the majority of subjects aged 25-35 years, 51,3%. The education level of most subjects about 57,1%, was a bachelor's degree,. Testosterone levels at the start of the study were 1,41±0,64 for DRSP/EE group and 1,32±0,48 for CRPN/EE group. For the degree of acne, 85.7% of subjects using DRPS/EE and 85% of subjects using CPRN/EE had reduced acne lesions with P value of 0.645. For hirsutism by calculating the FG score, 61.9% of the subjects using DRSP/EE and 50% of the subjects consumed CPRN/EE had a decreased FG score with a value of P value 0.443, and their menstrual period became regular for 85.7% of the subjects using DRSP/EE and 90% of the subjects using CPRN/EE with P value 0.645. Conclusion: Drospirenone and ethinyl estradiol are as effective as ciproterone acetate combined with etnyl estradiol in the treatment of moderate to severe acne vulgaris, hirsutism, secondary amenorrhea and oligomenorrhea.
