Presentasi Klinis Pertama Pasien Lupus Eritematosus Sistemik (LES) Di RSUPN Dr. Cipto Mangunkusumo = Initial Clinical Presentation of Systematic Lupus Erythematosus (SLE) Patients in Cipto Mangunkusumo Hospital.

Latar Belakang Lupus Eritematosus Sistemik (LES) merupakan penyakit autoimun sistemik yang memiliki karakteristik tanda dan gejala yang sangat beragam dan nonspesifik dengan presentasi klinis yang melibatkan lebih dari satu sistem organ. Hal ini seringkali menjadi tantangan dan berakibat pada keterlambatan diagnosis serta prognosis yang buruk. Oleh karena itu, penelitian dilakukan dengan tujuan untuk memahami presentasi klinis pertama pasien sebagai upaya meningkatkan deteksi dini dan menegakkan diagnosis awal. Metode Penelitian ini menggunakan desain studi potong lintang dengan metode deskriptif kuantitatif. Data yang digunakan adalah data sekunder berupa rekam medis pasien LES yang baru memulai kunjungan ke Poli Reumatologi di RSCM pada periode Mei 2021- Desember 2022. Subjek dipilih dengan metode total sampling dan data dianalisis untuk variabel usia, jenis kelamin, onset gejala, serta presentasi klinis pertama pasien (SPSS 25.0). Hasil analisis kemudian ditampilkan dalam bentuk tabel dan narasi deskriptif. Hasil Sebanyak 231 pasien terpilih berdasarkan kriteria inklusi dan eksklusi dengan 96,1% pasien adalah perempuan. Sebanyak 38,1% pasien berada dalam kelompok usia 21-30 tahun dan onset gejala paling banyak ditemukan pada kelompok 0-1 tahun sebesar 67,5%. Sekitar 24,2% pasien didapatkan mengalami manifestasi konstitusional dengan adanya keterlibatan organ lainnya, antara lain muskuloskeletal (87,4%), mukokutan (84%), hematologik (38,5%), ginjal (33,3%), dan neuropsikiatri (8,7%). Tidak ditemukan adanya keterlibatan paru, jantung, pembuluh darah, gastrointestinal dan hepatik, okular, obstetrik, serta endokrin. Dari 231 subjek, 44,2% di antaranya memiliki keterlibatan dua organ sekaligus yang muncul sebagai presentasi klinis pertama. Kesimpulan Keterlibatan muskuloskeletal dan mukokutan paling sering ditemukan sebagai presentasi klinis pertama pasien LES di RSCM yang diikuti oleh hematologik, ginjal, dan neuropsikiatri.

Introduction Systemic Lupus Erythematosus (SLE) is a systemic autoimmune disease characterized by highly diverse and nonspecific signs and symptoms with clinical presentations involving more than one organ system. This often poses a challenge and leads to delayed diagnosis and poor prognosis. Therefore, the study is conducted with the aim of understanding the patients’ initial clinical presentation in order to enhance early detection and establish an early diagnosis. Method A cross-sectional study was performed using a quantitative descriptive method. The data utilized consisted of secondary data from medical records of SLE patients who commenced their visits to the Rheumatology Clinic at RSCM from May 2021-December 2022. Subjects were selected using total sampling method, and the data were analyzed for age, gender, symptom onset, as well as the patients' initial clinical presentation (SPSS 25.0). The analysis results were then presented in the form of tables and descriptive narratives. Results A total of231 patients were selected based on inclusion and exclusion criteria, with 96,1% were females. A total of 38,1% of the patients belonged to the 21-30 age group, and symptom onset was most commonly observed in the 0-1 year age group, accounting for 67,5%. Approximately 24,2% of the patients exhibited constitutional manifestations with involvement of other organs, including musculoskeletal (87,4%), mucocutaneous (84%), hematologic (38,5%), renal (33,3%), and neuropsychiatric (8,7%) systems. Lungs, heart, blood vessels, gastrointestinal and hepatic systems, eyes, obstetric, and endocrine involvement was not found in this study. Among the 231 subjects, 44,2% of them had simultaneous involvement of two organs, which emerged as their initial clinical presentation. Conclusion Involvement of the musculoskeletal and mucocutaneous systems was most commonly observed as the initial clinical presentation of SLE patients at RSCM followed by hematologic, renal, and neuropsychiatric involvement.
