Perbandingan Luaran Fungsional pada Pasien yang Menggunakan Brace Lutut dan Tanpa Menggunakan Brace Lutut Pasca Rekonstruksi Ligamen Kruciatum Anterior Dengan Autograft Quadricep Tendon Per Artroskopi = Comparison of Functional Outcomes in Patients Using Knee Brace and Without Using Knee Brace Post Anterior Cruciate Ligament Reconstruction with Autograft Quadricep Tendon per Arthroscopy.

Latar Belakang: Penatalaksanaan cedera pada ACL harus dilakukan secara holistik dan komprehensif. Brace bertujuan untuk menjaga range of motion (ROM), memberikan stabilitas, serta mencegah cedera pada lutut. Banyak perdebatan dan kontroversi dalam penggunaan brace lutut pasca rekonstruksi ACL. Di Indonesia saat ini belum ada penelitian yang meneliti perihal penggunaan brace lutut pasca rekonstruksi ACL. Berdasarkan hal tersebut maka dalam penelitian ini telah diteliti apakah terdapat perbedaan luaran fungsional antara pasien yang menggunakan brace lutut dan tanpa menggunakan brace lutut pasca rekonstruksi ACL dengan bone quadricep tendon. Metode: Penelitian ini merupakan studi randomized controlled trial pada pasien dengan atau tanpa pemakaian brace di RS Cipto Mangunkusumo. Subjek penelitian dikumpulkan periode Februari 2022 hingga Februari 2023. Total subjek yang dikumpulkan dan dianalisis berjumlah 24 pasien. Luaran klinis sebelum dan sesudah rekonstruksi ACL dibandingkan meliputi skor International Knee Documentation Committee (IKDC), Tegner Lysholm, Knee Injury and Osteoarthritis Outcome Score (KOOS), Anterior Cruciate Ligament Return to Sport After Injury (ACL-RSI), dan rolimeter pada saat praoperasi, 1 bulan, dan 3 bulan. Hasil: Tidak ada perbedaan yang signifikan pada karakteristik subjek kedua kelompok dari jenis kelamin, sisi lutut, dan keterlibatan cedera meniskus. Pemeriksaan rollimeter menunjukan perbedaan yang signifikan pada bulan pertama dan ketiga pasca-operasi dimana kelompok dengan bracing menunjukan translasi yang lebih besar. Tidak terdapat perbedaan yang signifikan pada skoring luaran fungsional IKDC, KOOS dan Lysholm dengan p > 0,05 pada pre-operatif, 1 bulan, dan 3 bulan pasca operasi. Didapatkan perbedaan yang signifikan pada pemeriksaan rollimeter pada bulan ke 1 dan 3 (p=0,001 dan 0,003). Pada skoring ACL-RSI terdapat perbedaan bermakna dengan pada 3 bulan pasca operasi (p=0,004). Kesimpulan: Tidak terdapat perbedaan luaran fungsional antara pasien yang menggunakan brace lutut dan tanpa brace lutut pasca rekonstruksi ligamen kruciatum anterior dengan autograft bone quadricep tendon per artroskopi

Introduction: The management of ACL injuries should be done holistically and comprehensively. The brace aims to maintain range of motion (ROM), provide stability, and prevent injury to the knee. There is much debate and controversy in the use of knee braces post ACL reconstruction. In Indonesia, there is currently no research examining the use of knee braces after ACL reconstruction. Based on this, this study will examine whether there are differences in functional outcomes between patients who use knee braces and without using knee braces after ACL reconstruction with bone quadricept tendon. Method: This study is a randomized controlled trial study in patients with or without the use of braces at Cipto Mangunkusumo Hospital. The subjects of the study were collected in February 2022 until February 2023. Clinical outcomes before and after ACL reconstruction will be compared including International Knee Documentation Committee (IKDC) scores, Tegner Lysholm, Knee Injury and Osteoarthritis Outcome Score (KOOS), Anterior Cruciate Ligament Return to Sport After Injury (ACL-RSI), and rollimeter at pre-operative time, 1 month, and 3 months. Result: There were no significant differences in the subjects characteristics of both groups of sex, knee side, and meniscus injury involvement. The rollimeter examination showed significant differences in the first- and third-months post-surgery where the group with bracing showed greater translation. There was no significant difference in the functional output scoring of IKDC, KOOS, Lysholm, with p > 0.05 at pre-operative, 1 month, and 3 months postoperatively. There was a significant difference in the rollimeter examination at 1 and 3 months (p=0.001 and 0.003). There was a significant difference in the ACL-RSI scoring with 3 months postoperatively (p=0.004). Conclusion: There was no difference in functional outcomes between patients using a knee brace and without a knee brace after anterior cruciate ligament reconstruction with quadricep tendon autograft per arthroscopy
