Uji Validitas Stetoskop Digital Stemoscope sebagai Alat Bantu Diagnostik Tuberkulosis = Validity of Digital Stethoscope Stemoscope as a Tuberculosis Diagnostic Aid.

Latar belakang: Beban tuberkulosis (TB) Indonesia yang tinggi semakin memburuk dengan keadaan pandemi yang mengurangi angka notifikasi TB. Salah satu penyebab utama adalah keresahan masyarakat untuk mendatangi fasilitas kesehatan dengan adanya resiko penularan yang tinggi. Pemeriksaan diagnostik TB memerlukan pasien untuk datang berulang kali ke fasilitas kesehatan. Selain itu, belum ada metode untuk menyimpan atau membagikan data auskultasi dengan menggunakan stetoskop konvensional. Penggunaan stetoskop digital Stemoscope untuk mendiagnosis TB dengan mengidentifikasi suara paru khusus dapat menjadi solusi untuk kedua masalah tersebut. Metode: Penelitian ini membandingkan gejala ronki saat auskultasi Stemoscope dengan hasil test cepat molekuler (TCM) Genexpert sebagai baku emas. Rekaman suara paru diambil dari pasien terduga TB yang mendatangi Rumah Sakit Umum Pusat Persahabatan (RSUPP) selama Oktober 2022. Rekaman ini kemudian diinterpretasikan oleh dokter spesialis paru. Pasien juga memberikan sampel sputum untuk dilakukan pemeriksaan laboratorium. Data hasil diagnosis test cepat molekuler (TCM) Genexpert dikumpulkan kemudian hari. Hasil: Penelitian mendapatkan 25 pasien terduga TB dengan 13 terkonfirmasi positif TB dan 12 negatif TB berdasarkan hasil TCM GeneXpert. Dari sampel tersebut, 13 memiliki suara nafas tambahan ronki dan 12 pasien lainnya tidak. Didapatkan nilai sensitivitas Stemoscope 62% sedangkan spesifisitas 58% Kesimpulan: Stemoscope tidak valid digunakan sebagai alat bantu diagnostik TB berdasarkan kehadiran suara nafas ronki. Penelitian lebih lanjut dapat dirancang untuk meminimalkan faktor perancu dan/ integrasi dengan klasifikasi otomatis artificial intelligence.

Introduction: Indonesia’s high tuberculosis (TB) burden worsens during the COVID-19 pandemic with the decrease of TB case notification. One of the main causes is public unrest to visit health facilities with a high risk of transmission. The many diagnostic tests of TB require patients to repeatedly visit the health facility. Furthermore, there is no method to store or share auscultation data using a conventional stethoscope. Use of digital stethoscope Stemoscope to diagnose TB by identifying specific lung sounds can be a solution to both problems. Method: This study compares the performance ofStemoscope with GeneXpert molecular test as a golden standard. A positive TB result from lung auscultation is given if a rhonchi and/ crackles sound is detected. Lung sound recordings are taken from suspected TB patients that visited Persahabatan General Hospital during October 2022. The recordings are later interpreted by a specialist. The patients are also asked to give a sputum sample for a lab test. The diagnostic results of GeneXpert are later compiled. Result: A total of 25 patients are accumulated for this study where 13 are confirmed positive TB and 12 are negative TB based on GeneXpert results. From those samples, 13 patients had additional rhonchi and crackle sounds while the other 12 patients did not. The Stemoscope sensitivity value is 62% while the specificity is 58%. Conclusion: Stemoscope is not valid to be used as a TB diagnostic aid based on the presence of rhonchi and crackle breath sounds. Further research can be designed to minimize confounding factors and/or integration with artificial intelligence automatic classification.
