Factors Affecting the 1 -year Survival Rate of Patients Undergoing Elective Percutaneous Coronary Intervention: A Single Center Experience.

Background: Globally, cardiovascular diseases are acknowledged to be the leading cause of death, reaching 55.9%, 38.7% and 31.7% in America, Japan and Singapore, respectively. Nationally, 4.2 million people experience cardiovascular diseases, in which 50% are at risk of experiencing fatal cardiac arrest. Percutaneous Coronary Intervention (PCI) acts as a reperfusion therapy for CAD patients and its one-year survival rate is affected by risk factors expressed by patients and is scarcely studied in Indonesia. Method: A retrospective cohort study was conducted by looking through the PCI registry of patients who had undergone elective PCI at RSCM between January 201 7 to December 2018. Patients were then observed after one year of elective PCI for their survival outcomes. The survival analysis was performed using a Kaplan-Meier method, followed by cox regression assess its significance. Results: In this research, there were 329 patients studied. The one-year survival rate of patients undergoing elective PCI in Dr Cipto Mangukusumo Hospital was 96.7%. The mean age of these patients was 60.14 ± 10.048, showing a baseline characteristic of >= 60 years old (50.2%), male (82.1%), obesity (59%), hypertension (55.9%), diabetes mellitus (51.7%) and dyslipidemia (53.2%). The variables of gender (female), obesity, hypertension, diabetes mellitus and dyslipidemia affected one-year survival with an HR of 2.63 (CI95% 0.77-8.98), 1.23 (CI95% 0.74-4.20), 2.10 (CI95% 0.56-7.92), 2.52 (0.67-9.48), 1.53 (CI95% 0.45-5.24), respectively. Conclusion: The one-year survival of patients after undergoing elective PCI in Cipto Mangukusumo Hospital was 96.7% while expressing no significant difference between risk factors and the outcome of elective PCI.

Latar Belakang: Secara global, penyakit jantung diakui sebagai penyebab kematian utama, mencapai 55.9%, 38.7% dan 31.7% di Amerika, Jepang dan Singapura. Di Indonesia, 4,2 juta orang mengalami penyakit jantung, dimana 50% dari mereka berisiko akan serangan jantung fatal. Sebagai prosedur intervensi revaskularisasi, Intervensi Koroner Perkutan (IKP) elektif merupakan salah satu terapi reperfusi untuk pasien penyakit jantung koroner. Kesintasannya dalam satu tahun dipengaruhi oleh beberapa faktor risiko yang diekspresikan oleh pasien dan jarang dipelajari di Indonesia Metode: Sebuah studi retrospektif kohort telah dilakukan dengan melihat PCI registry untuk pasien-pasien yang telat mengalami IKP elektif di RSCM diantara Januari 201 7 – Desember 2018. Pasien lalu diobservasi selama satu tahun untuk melihat kesintasan mereka. Analisa kesintasan dilakukan dengan metode Kaplan Meier, dilanjutkan dengan Log-Rank Test untuk melihat makna di korelasi tersebut. Hasil: Pada studi ini, terdapat 329 pasien yang dianalisa. Kesintasan satu tahun pada pasienpasien yang sudah mengalami IKP elektif di RSCM mencapai 96.7%. Usia rata-rata mereka adalah 60.14 ± 10.048, dengan karakteristik >=60 tahun (50.2%), laki-laki (82.1%), obesitas (59%), hipertensi (55.9%), diabetes melitus (51.7%) dan dislipidemia (53.2%). Terlihat, variabel jenis kelamin (perempuan), obesitas, hipertensi, diabetes melitus dan dislipidemia mempengaruhi kesintasan satu-tahun dengan HR 2.63 (CI95% 0.77-8.98), 1.23 (CI95% 0.74- 4.20), 2.10 (CI95% 0.56-7.92), 2.52 (0.67-9.48), 1.53 (CI95% 0.45-5.24), masing-masing. Kesimpulan: Kesintasan satu tahun pada pasien-pasien yang sudah mengalami IKP elektif di RSCM adalah 96.7% sementara menyatakan tidak ada perbedaan bermakna antara factor risiko dan hasil IKP elektif.
