Gambaran Implementasi Rekomendasi IBS-GEC ESTRO-ABS 2021 Dalam Delineasi IGABT Pada Pasien Kanker Serviks = An overview of the implementation of the IBS-GEC ESTRO-ABS 2021 recommendations in the delineation of IGABT in cervical cancer patients.

Tujuan: Mengetahui keselarasan dalam delineasi brakiterapi 3D dan mencari tahu apakah gambaran delineasi dari rekomendasi delineasi brakiterapi 3D oleh IBS-GEC ESTRO-ABS 2021 dapat menjadi model prediksi respon terapi pada pasien kanker serviks yang menjalani brakiterapi 3D dengan modalitas CT scan serta mendapatkan cutoff volume HRCTV yang menjadi indikasi pemberian tambahan interstitial pada brakiterapi intrakaviter. Metodologi: Penelitian merupakan studi case control. Melakukan delineasi ulang HRCTV dengan rekomendasi IBS-GEC ESTRO-ABS 2021 pada 130 pasien kanker serviks yang mendapat brakiterapi 3D di RSUPN Cipto Mangunkusumo tahun 2018-2020 lalu dilakukan penghitungan DSC serta analisis variabel pada respon terapi dan cut-off volume HRCTV. Hasil: Delineasi pada 374 fraksi brakiterapi 3D mendapatkan rerata nilai kemiripan/ DSC 74%. Adanya perbedaan volume HRCTV antar kelompok respon terjadi pada kelompok DSC ≥ 80% (p=0.0129). Tidak terdapat perbedaan volume & dosis D90 HRCTV lama pada pada kelompok respon komplit dengan respon tidak komplit (p=0.075 & p=0.22) sedangkan terdapat perbedaan bermakna pada HRCTV rekomendasi (p=0.0068 & p=0.0403). Analisis multivariat mendapatkan histologi, kemoradiasi dan kriteria brakiterapi menjadi prediktor independen yang mempengaruhi respon terapi (p=0.03, p=0.043 & p=0.000). Cut-off HRCTV diambil dari HRCTV lama pada kelompok DSC ≥ 80% mendapatkan HRCTV 43.29 cc dengan sensitivitas 100% dan spesifisitas 77%. Kesimpulan: Delineasi yang dilakukan dengan nilai kemiripan (DSC) minimal 80% dengan delineasi rekomendasi IBS-GEC ESTRO-ABS 2021 dapat memberikan gambaran perbedaan volume antar kelompok respon terapi yang sejalan dengan penelitian yang sudah ada dan dengan menggunakan pemetaan delineasi rekomendasi dapat ditemukan cut-off volume HRCTV 43.29 cc sebagai batas volume indikasi penambahan interstitial pada brakiterapi intrakaviter sehingga volume pada HRCTV yang besar bisa mendapatkan sebaran dosis yang lebih dan dengan total dosis lebih tinggi tanpa meningkatkan toksisitas OAR.

Aims: To find out the alignment in the delineation of 3D brachytherapy and whether the delineation of the recommended 3D brachytherapy by IBS-GEC ESTRO-ABS 2021 can be a predictive model of therapy response in cervical cancer patients undergoing 3D brachytherapy with CT scan modalities and get a cut-off HRCTV volume which is an indication for interstitial adjunct in intracavitary brachytherapy. Methodology: This is a case-control study. Re-delineating HRCTV with IBS-GEC ESTRO-ABS 2021 recommendations on 130 cervical cancer patients who received 3D brachytherapy at Cipto Mangunkusumo Hospital in 2018-2020 and then carried out DSC calculations and analysis of variables on therapy response and cut-off volume of HRCTV. Results: Delineation of 374 3D brachytherapy fractions obtained an average similarity value/DSC of 74%. The difference in HRCTV volume between response groups occurred in the DSC group at 80% (p=0.0129). There was no difference in volume & dose of old D90 HRCTV in the complete response group with incomplete response (p = 0.075 & p = 0.22) while there was a significant difference in the recommended HRCTV (p = 0.0068 & p = 0.0403). Multivariate analysis found that histology, chemoradiation, and brachytherapy criteria were independent predictors that affected the response to therapy (p=0.03, p=0.043 & p=0.000). The cut-off of HRCTV was taken from the old HRCTV in the DSC group at 80% and got 43.29 cc of HRCTV with a sensitivity of 100% and a specificity of 77%. Conclusion: The delineation carried out with a similarity value (DSC) of at least 80% with the IBS-GEC ESTRO-ABS 2021 recommendation can provide an overview of the difference in volume between therapy response groups which is in line with existing research and by using the recommendation delineation mapping can be found cut-off off the volume of 43.29 cc HRCTV as a volume limit indication of interstitial addition to intracavitary brachytherapy so that large volumes of HRCTV can get more dose distribution and with a higher total dose without increasing OAR toxicity.
