Hubungan Derajat Anemia dengan Spesies, Densitas, dan Stadium Plasmodium pada Malaria = The Relationship of Degree of Anemia with Plasmodium Species, Density, and Stage in Malaria.

Latar belakang: Anemia pada malaria merupakan hasil dari invasi Plasmodium ke dalam eritrosit. Derajat anemia bergantung pada aspek parasit dan inang. Aspek parasit: spesies, stadium dan densitas. Aspek inang berupa meningkatnya TNF-α dan IL-6 yang berpengaruh dengan kejadian anemia. Beberapa penelitian yang menghubungkan pengaruh aspek parasit terhadap anemia masih menunjukkan hasil yang berbeda-beda. Tujuan: untuk mengetahui pengaruh parasit malaria (spesies, stadium dan densitas) terhadap anemia. Metode: Penelitian analitik observasional dengan desain Cross Sectional. Data bersumber dari penelitian utama yang telah dilakukan di daerah endemik malaria. Sebanyak 99 subjek positif malaria menjadi sampel penelitian ini. Status malaria, spesies, densitas, dan stadium ditetapkan berdasarkan pemeriksaan mikroskopis pada sediaan darah tebal atau tipis yang diwarnai dengan giemsa. Kadar hemoglobin diukur menggunakan mesin symex. Hasil: Pada P. vivax yang berjumlah 50,5% subjek, ditemukan kadar Hemoglobin lebih rendah (10,86 g/dl) dibandingkan kelompok yang terinfeksi P. falciparum sejumlah 49,5% (11,11 g/dl), tetapi perbedaan kadar ini tidak signifikan (p = 0,296). Perbandingan rerata kadar hemoglobin pada status gametosit menunjukkan hasil yang signifikan lebih rendah (p=0,003) pada kelompok dengan gametosit positif (10,24 g/dl) dibandingan kelompok gametosit negatif (11,39 g/dl). Hasil uji Spearman antara densitas dengan nilai hemoglobin menunjukkan korelasi negatif yang signifikan (p = 0,031; r= -0,188). Kesimpulan: Tidak terdapat perbedaan kadar Hb pada subjek yang terinfeksi P. falciparum dan P. vivax. Kadar Hb yang signifikan lebih rendah ditemukan pada subjek yang sediaannya ditemukan stadium gametosit dan densitas parasit yang tinggi.

Introduction: Anemia in malaria is the result of Plasmodium invasion into erythrocytes. The degree of anemia depends on the parasitic and host aspect. The parasitic aspect: species, stadium, and density. The host aspect, indicated by increasing TNF-α and IL-6 affects the incidence of anemia. Several studies linking the influence of parasitic aspects on anemia still show different results. Purpose: To determine the effect of malaria parasites (species, stage, and density) on anemia. Method: Analytical observational study with a cross-sectional design. Data sourced from the main research that has been carried out in malaria-endemic areas. A total of 99 subjects who were malaria positive were sampled in this study. Malaria status, species, stadium, and density were determined based on microscopic examination of thick or thin blood colored with giemsa. Hemoglobin levels were measured using the symex machine. Result: In P. vivax infection which amounted to 50.5% of subjects, hemoglobin levels were found to be lower (10.86 g/dl) than the 49.5% P. falciparum-infected group (11.11 g/dl), but this difference in levels not significant (p = 0.296). Comparison of the mean hemoglobin level on gametocyte status showed a significantly lower result (p=0.003) in the group with positive gametocytes (10.24 g/dl) than in the negative gametocyte group (11.39 g/dl). The results of the Spearman test between density and hemoglobin value showed a significant negative correlation (p = 0.031; r = -0.188).. Conclusion: There is no difference in Hemoglobin levels in subjects infected with P. falciparum and P. vivax. Significantly lower Hb levels were found in subjects with gametocyte stage and high parasite density.
