VALIDASI SKOR MASCC DAN NILAI TAMBAH PROKALSITONIN DALAM MEMPREDIKSI KOMPLIKASI MEDIS SERIUS PASIEN DEMAM NEUTROPENIA TIDAK TERKAIT KEMOTERAPI = VALIDATION OF MASCC SCORE AND PROCALSITONIN ADDED VALUE IN PREDICTING SERIOUS COMPLICATIONS FOR FEBRILE NEUTROPENIA PATIENTS NOT RELATED TO CHEMOTHERAPY.

Latar Belakang. Pasien demam neutropenia yang bukan akibat efek samping kemoterapi angka kejadiannya semakin meningkat. Kondisi neutropenia meningkatkan risiko terjadinya infeksi, adapun penyebab neutropenia tidak hanya obat kemoterapi. Beberapa sistem penilaian risiko komplikasi digunakan untuk dasar terapi antibiotik pada pasien demam neutropenia pasca kemoterapi, belum ada penelitian terhadap pasien demam neutropenia selain pasca kemoterapi. Penelitian ini bertujuan untuk validasi dan menilai performa skor MASCC dan penambahan prokalsitonin pada populasi demam neutropenia tidak terkait kemoterapi. Metode. Penelitian dilakukan pada rekam medis 68 pasien demam neutropenia yang bukan terkait kemoterapi di ruang perawatan RSUP Nasional dr. Cipto Mangunkusumo pada bulan Januari 2018-November 2019. Dilakukan penilaian validasi kalibrasi menggunakan HosmerLameshow dan deskripsi dengan ROC. Hasil. Terdapat variasi skor MASCC dan nilai prokalsitonin subjek penelitian. Median skor MASCC adalah 21 (18 – 24) yang masuk dalam kategori low-risk. Median prokalsitonin subjek adalah 1,99 ng/mL (0,72 – 10,60). Performa MASCC menghasilkan kalibrasi baik p > 0,05 dan area under curve (AUC) sebesar 0,888(IK95% 0,813 – 0,962, p = 0,000). Prokalsitonin menunjukkan AUC sebesar 0,797 (IK95% 0,683 – 0,911, p = 0,000), titik potong 1,67 dengan sensitifitas 78,8% dan spesifisitas 72,4%. Performa gabungan skor MASCC dan prokalsitonin juga menghasilkan kalibrasi Hosmer-Lameshow dengan p > 0,05 dan AUC sebesar 0,901 (IK95% 0,827 – 0,974). Kesimpulan. Dapat disimpulkan bahwa MASCC merupakan instrumen yang baik untuk mendeteksi komplikasi perawatan pada pasien dengan FN yang tidak terkait kemoterapi. Gabungan antara MASCC dan PCT didapatkan hasil yang bermakna dalam prediktor komplikasi dengan validasi baik.

Background. Neutropenia patients who are not due to side effects of chemotherapy are increased. Neutropenia increases the risk of infection. Several complications risk assessment systems are used for antibiotic therapy in patients with post-chemotherapy neutropenia. There are no studies in neutropenic fever patients who are not due to chemotherapy drugs. This study aims to validate and assess the performance of the MASCC score and the addition of procalcitonin in the neutropenia fever population not related to chemotherapy. Methods. The study was conducted on the medical records of 68 neutropenic fever patients who were not chemotherapy-related in the National Hospital Dr. Cipto Mangunkusumo in January 2018-November 2019. The calibration validation was assessed using HosmerLameshow and description with ROC. Results. There are variations in MASCC scores and procalcitonin scores in the study subjects. The median MASCC score is 21 (18-24). The median procalcitonin subject was 1.99 ng / mL (0.72 - 10.60). MASCC's performance resulted in a good calibration of p > 0.05 and area under curve (AUC) of 0.888 (IK95% 0.813 - 0.962, p = 0.000). Procalcitonin showed AUC of 0.797 (IK95% 0.683 - 0.911, p = 0.000), the cut point 1.67 with a sensitivity of 78.8% and specificity of 72.4%. The combined performance of MASCC and procalcitonin scores also resulted in Hosmer-Lameshow calibration with p > 0.05 and AUC of 0.901 (IK95% 0.827 - 0.974). Conclusion. It can be concluded that MASCC is a good instrument for detecting treatment complications in patients with FN that are not chemotherapy related. The combination with PCT has significant results in predictors of complications with good validation.
