Kadar Nitrit Oksida Udara Napas Pada Pasien PPOK Stabil Disertai Pemeriksaan Hitung Eosinofil Di Rumah Sakit Rujukan Respirasi Nasional Persahabatan = Exhaled Nitric Oxide Level and Blood Eosinophil Count of Stable Chronic Obstructive Pulmonary Disease Patients Treated at National Referral Respiratory Center Persahabatan Hospital Jakarta, Indonesia.

Latar Belakang : Penyakit paru obstruktif kronik (PPOK) memiliki hambatan aliran napas bersifat progresif dan persisten yang didapat dari respon inflamasi kronik sehingga kemudian menghasilkan Nitrit Oksida (NO) respirasi yang dapat diukur. Kadar NO ekspirasi berfungsi sebagai penanda inflamasi untuk memonitor derajat inflamasi suatu penyakit dan respon terapi. Tujuan : Penelitian ini bertujuan untuk mengetahui rerata kadar NO ekspirasi serta mengetahui hubungan antara kadar NO ekspirasi dan hitung eosinofil pada pasien PPOK stabil. Metode : Penelitian ini merupakan penelitian potong lintang yang dilakukan di RS Rujukan Respirasi Nasional Persahabatan pada bulan Februari - April 2019 untuk melihat kadar NO ekspirasi pada pasien PPOK stabil. Pemilihan secara consecutive mendapatkan 53 pasien sebagai subjek penelitian. Hasil dari wawancara, pemeriksaan fisis, pemeriksaan uji faal paru, pemeriksaan kadar NO ekspirasi dan pemeriksaan laboratorium dari setiap subjek dicatat dan dianalisis. Hasil : Dalam penelitian ini didominasi dengan subjek laki-laki (86.79%) dengan rerata usia 63.45±8.53 tahun. Rerata kadar NO ekspirasi pada penelitian ini adalah 18 ppb. Rerata kadar NO ekspirasi pada kelompok PPOK non eosinofilik (58.5%) sebesar 17 ppb dan pada kelompok PPOK eosinofilik (41.5%) sebesar 22.5 ppb. Terdapat perbedaan bermakna kadar NO ekspirasi pada dua kelompok tersebut. Tidak terdapat hubungan antara kadar NO ekspirasi dengan hitung eosinofil dan riwayat merokok. Kesimpulan : Rerata nilai kadar NO ekspirasi pada kelompok PPOK stabil adalah 18 ppb. Terdapat perbedaan bermakna pada kadar NO ekspirasi antara kelompok PPOK non eosinofilik dan PPOK eosinofilik.

Background: Chronic obstructive pulmonary disease (COPD) is characterized by persistent and progressive airflow limitation due to chronic inflammatory response, which results in the release of detectable exhaled nitric oxide (NO). The exhaled NO acts as an inflammation biomarker in monitoring the severity degree of the disease and the therapeutic response. Objective: This study aimed to measure the exhaled NO level and to reveal the correlation between exhaled NO level and blood eosinophil count in stable COPD patients. Methods: This cross-sectional study included stable COPD patients treated at National Referral Respiratory Center Persahabatan Hospital Jakarta, Indonesia, between February - April 2019. Consecutive sampling resulted in the recruitment of 53 patients as subjects. Results of the interview, physical examination, lung function test, fractional concentration of exhaled NO test, and laboratory test of the subjects were recorded and analyzed. Results: This study revealed that subjects were predominantly males (86.79%), with the mean age of 63.45±8.53 years old. The mean exhaled NO level of the subject was 18 ppb. The mean exhaled NO level of non-eosinophilic COPD (58.5%) was 17 ppb, and of eosinophilic COPD (41.5%) was 22.5 ppb. There was a significant difference in the exhaled NO level between the two groups. There was no correlation between exhaled NO level with blood eosinophil counts and smoking history. Conclusion: The mean exhaled NO level in stable COPD patients was 18 ppb. There was a significant difference in exhaled NO level between the non-eosinophilic and eosinophilic COPD.
