Aesthetic Outcome Using Asher McDade Scoring System for The Unilateral Cleft Lip Patient That Has Been Repaired with the Modified Rotation-Advancement Gentur’s Method and Fisher Techniques = Keluaran Estetika Berdasarkan Sistem Skoring Asher McDade Untuk Pasien Sumbing Bibir Unilateral yang Dioperasi dengan Metode Rotation-Advancement Modifikasi Gentur dan Teknik Fisher.

Background: As a national referral hospital, Cipto Mangunkusumo Hospital, the division of plastic surgery has been treating many congenital anomaly cases. One of the most common congenital anomalies that can be found in the plastic surgery division are cleft lip and palate. There are two mostly used technique to operate patient with unilateral cleft lip. Rotation-advancement method by Gentur based on Millard technique and The Fisher technique repair. This study will compare the aesthetic outcome of those techniques using Asher McDade scoring system. Methods: Thirty two patients with unilateral cleft lip operated by two senior boardcertified surgeons, using rotation-advancement technique by Gentur and Fisher technique then took postoperative photographs at least 6 months after the surgery. Four observers will evaluate the patient with Asher-McDade scoring system to compare aesthetic outcome of those operation techniques. Results: There is no significant differences between Gentur and Fisher groups in gender. There is a significant difference in age at repair between both method (p=0,032) with the Fisher method is predominantly done in the patient with age 3-6 month old. The intra-rater reliability for overall score has good agreement for rater 1 (ICC = 0,672), and excellent agreement for rate 2, 3, and 4 (ICC = 0,842; 0,828; 0,883 accordingly). The inter-rater reliability in overall score are in a good agreement (ICC = 0,668). based on the rater 2, 3, and 4, there are no significant Asher McDade score differences between two groups (p = 0,221; 0,144; 0,472 respectively). Conclusion: This study shows a good to excellent agreement in intra-rater reliability and good agreement in inter-rater reliability between the observer that assess the aesthetic outcome for the cleft lip repair patient. There is no aesthetic outcome difference between Gentur’s method and Fisher technique in cleft lip repair evaluated by Asher McDade scoring system.

Latar Belakang: Sebagai rumah sakit rujukan nasional, divisi bedah plastik Rumah Sakit Cipto Mangunkusumo, menangani banyak kasus anomali kongenital. Salah satu anomali bawaan yang paling umum adalah bibir sumbing dan langit-langit. Ada dua teknik yang paling banyak digunakan untuk mengoperasi pasien dengan bibir sumbing unilateral, yaitu teknik rotation-advancement oleh Gentur berdasarkan teknik Millard dan teknik Fisher. Penelitian ini akan membandingkan hasil estetika dari kedua teknik tersebut menggunakan sistem penilaian Asher McDade. Metode: Tiga puluh dua pasien dengan bibir sumbing unilateral dioperasi oleh dua ahli bedah senior bersertifikat, menggunakan teknik rotation-advancement oleh Gentur atau teknik Fisher kemudian dilakukan pengambilan foto pasca operasi setidaknya 6 bulan setelah operasi. Empat penilai akan mengevaluasi pasien dengan sistem penilaian Asher-McDade untuk membandingkan hasil estetika dari kedua teknik operasi tersebut. Hasil: Tidak ada perbedaan kelamin yang signifikan antara kelompok Gentur dan Fisher. Terdapat perbedaan yang signifikan dalam usia saat operasi antara kedua metode (p = 0,032) dimana pada teknik Fisher, operasi lebih sering dilakukan pada pasien dengan usia 3-6 bulan. Keandalan intra-penilai untuk skor keseluruhan memiliki kesepakatan yang baik pada penilai 1 (ICC = 0,672), dan kesepakatan yang sangat baik untuk penilai 2, 3, dan 4 (ICC = 0,842; 0,828; 0,883 secara berurutan). Keandalan antar penilai dalam skor keseluruhan memiliki nilai kesepakatan yang baik (ICC = 0,668). Berdasarkan penilai 2, 3, dan 4, tidak ada perbedaan skor Asher McDade yang signifikan antara dua kelompok (p = 0,221; 0,144; 0,472 secara berurutan). Kesimpulan: Penelitian ini menunjukkan kesepakatan yang baik hingga sangat baik dalam reliabilitas intra-penilai dan kesepakatan yang baik dalam reliabilitas antarpenilai dalam mengevaluasi hasil estetika dari pasien yang dilakukan operasi perbaikan bibir sumbing. Tidak ada perbedaan hasil estetika antara metode Gentur dan teknik Fisher dalam perbaikan bibir sumbing yang dievaluasi oleh sistem penilaian Asher McDade.
