Evaluation of Open Pyeloplasty in Patient with Uretero-Pelvic Junction Obstruction in Harapan Kita Hospital between 2014-2017 = Evaluasi Pieloplasti Terbuka pada Pasien dengan Obstruksi Ureteroperlvic Junction di Rumah Sakit Harapan Kita antara 2014-2017 .

UPJ obstruction defined as narrowing of uretero-pelvic junction that obstructs urine flow from renal pelvis to proximal ureter. Open pyeloplasty is one of the most common techniques used to treat hydronephrosis caused by UPJ obstruction. The aim of this study is to evaluate success rate and complications post pyeloplasty in Urology Department of Harapan Kita Mother and Children Hospital between 2014 to 2017. This is a descriptive study with retrospective approach. Data were collected from medical records of pediatric patients diagnosed with either unilateral or bilateral UPJObstruction underwent open pyeloplasty between 2014 to 2017 in Harapan Kita Mother and Children Hospital. From 16 patients, 10 were boys and 6 were girls, with 56.25% aged 30 mm in 81.25% patients, and 87.50% patients were operated due to progressive hydronephrosis. Preoperative nephrostomy were done in 62.5% patients, and all patients undergone the same Anderson-Hynes pyeloplasty technique with insersion of double J stents. Complications after surgery were fever (15.7%) and urosepsis (10.5%). Post-operative pyelum diameter after double J stent removal were > 30 mm in 1 & 3 month, and < 20 mm in 6 months(77.78%). Anderson-Hynes Pyeloplasty (Dismembered pyeloplasty) is the preferred technique used to treat UPJ obstruction with 94.8% success rate, with fever as the most common complication. Research with more samples and evaluation time is needed to confirm conclusion of this study.

Obstruksi ureteropelvico junction (UPJ) didefinisikan sebagai hambatan aliran urin dari pelvis ginjal ke ureter proksimal. Pieloplasti terbuka meurpakan salah satu teknik yang paling sering digunakan untuk mengatasi hidronefrosis yang disebabkan obstruksi UPJ. Tujuan dari penelitian ini untuk mengevaluasi tingkat kesuksesan dan komplikasi pasca pieloplasti di departemen urologi RSAB Harapan Kita antara tahun 2014 sampai 2017. Ini merupakan penelitian deskriptif dengan pendekatan retrospektif. Data dikumpulkan dari rekam medis pasien anak yang didiagnosa dengan unilateral atau bilateral obstruksi UPJ yang dilakukan pieloplasti terbuka antara 2014-2017 di RSAB Harapan Kita. Dari 16 pasien, 10 pasien anak laki laki dan 6 pasien anak perempuan, dengan 56,25% berusia kurang dari 1 tahun. Semua pasien didiagnosa dengan USG, 81,25% menunjukkan unilateral obstruksi UPJ. Preoperative diameter pyelum >30mm pada 81,25% pasien, dan 87,50% pasien di operasi karena hidronefrosis yang progresif. Preoperatif nephrostomy dilakukan pada 62,5% pasien, dan semua pasien yang dilakukan teknik Anderson-Hynes pyeloplasty, dilakukan insersi DJ stent. Komplikasi setelah operasi berupa demam (15,7%), dan urosepsis (10,5%). Diameter pyelum postoperative setelah pelepasan DJ stent sebesar >30 mm pada bulan ke 1 dan ke 3, dan < 20 mm pada bulan ke 6 (77.78%). Anderson-Hynes Pyeloplasty (Dismembered pyeloplasty) merupakan teknik yang sering dipilih dalam mengeatasi obstruksi UPJ dengan 94% tingkat kesuksesannya. Komplikasi yang paling sering muncul adalah demam. Penelitian lebih lanjut dengan sampel penelitian yang lebih banyak dan periode evaluasi yang lebih panjang diperlukan untuk konfirmasi kesimpulan dari penelitian ini.
