Perbedaan Imunoekspresi Fibroblast Growth Factor Receptor-3 (FGFR3) pada Karsinoma Urotelial Kandung Kemih Derajat Rendah dan Derajat Tinggi, serta Hubungannya dengan Stadium Tumor = The differentation of Fibroblast Growth Factor Receptor-3 (FGFR3) immunoexpression between low and high grade urothelial carcinoma of the bladder related to tumor stages.

Latar belakang: Karsinoma urotelial merupakan keganasan primer yang paling sering terjadi pada kandung kemih. Salah satu gangguan genetik yang sering ditemukan adalah pada gen FGFR3. Peningkatan ekspresi FGFR3 berhubungan dengan adanya mutasi, tetapi dapat juga terjadi akibat gangguan lain seperti fusi protein FGFR3. Ekspresi FGFR3 yang tinggi berpotensi menjadi target terapi yang menjanjikan terutama untuk kelompok muscle invasive bladder cancer (MIBC) yang memiliki kesintasan yang buruk. Tujuan penelitian ini membandingkan ekspresi FGFR3 pada karsinoma urotelial kandung kemih derajat rendah dan tinggi, serta kelompok non-muscle invasive bladder cancer (NMIBC) dan MIBC. Metode: Penelitian ini menggunakan desain potong lintang. Sampel terdiri atas 47 kasus karsinoma urotelial kandung kemih dengan berbagai derajat keganasan dan stadium dari arsip Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia/Rumah Sakit Cipto Mangunkusumo (FKUI/RSCM) tahun 2009 sampai 2017. Penelitian dilakukan dengan pemeriksaan imunohistokimia FGFR3, serta penghitungan H-score dan persentase ekspresi FGFR3 setiap kasus. Sampel dibagi menjadi dua kelompok yaitu kelompok non-overexpression (ekspresi ≤70%) dan kelompok overexperssion (ekspresi >70%). Hasil: Penelitian ini tidak mendapatkan adanya perbedaan ekspresi FGFR3 yang bermakna antara karsinoma urotelial kandung kemih derajat rendah dan derajat tinggi, serta antara kelompok NMIBC dan MIBC (p>0.05). Kesimpulan: Ekspresi FGFR3 tidak memiliki hubungan bermakna dengan derajat keganasan maupun stadium tumor (p>0.05).

Background: Urothelial carcinoma is the most common primary neoplasm in the bladder. Alteration in FGFR3 gene is frequently found in this neoplasm. FGFR3 overexpression is related to mutational status, but also other alterations, such as FGFR3 fusion protein. The overexpression of FGFR3 is potential to use as targeted therapy especially for muscle invasive bladder cancer (MIBC) which has low survival rate. The aim of this study is to compare FGFR3 expression between grades and stages of urothelial carcinoma of the bladder. Methods: This study was a crosssectional study. The samples were 47 cases of urothelial carcinoma from various grades and stages. The samples were taken from Department of Anatomical Pathology, Faculty of Medicine Universitas Indonesia/Rumah Sakit Cipto Mangunkusumo (FKUI/RSCM) from 2009 to 2017. All cases were examined with FGFR3 immunohistochemistry and counted for histo-score and percentage of FGFR3 expression. The samples were divided into 2 groups based on the level of FGFR3 expression. The first was non-overexpression group (score ≤70%) and the second was overexpression group (score >70%). Result: There was no significant difference of FGFR3 expression between low and high grade urothelial carcinoma of the bladder, and between non-muscle invasive bladder cancer (NMIBC) and MIBC groups (p>0.05). Conclusion: FGFR3 expression was not significantly associated with tumor grades and stages in urothelial carcinoma of the bladder (p>0.05).
