Pengaruh Aktivitas Fisik Berlebih (Active) Dan Faktor-Faktor Lainnya Terhadap Timbulnya Penyakit Osteoarthritis Genu Pada Prajurit Militer Laki-Laki Di Jakarta = The Effect Of Excessive Physical Activity (Active) And Other Factors Against On The Occurence Of Osteoarthritis Genu Disease In Male Military Soldiers In Jakarta.

LATAR BELAKANG : Saat ini, penyakit arthritis menjadi penyebab teratas dari kecacatan di antara prajurit veteran di Amerika Serikat dan menjadi alasan utama prajurit dinonaktifkan. Radang sendi terjadi karena berbagai hal. Salah satunya disebabkan oleh aktivitas yang sering dilakukan. Berbagai aktivitas tersebut menyebabkan sendi mengalami penekanan secara terus menerus yang pada akhirnya berakibat pada kerusakan dan peradangan. Aktivitas fisik juga dapat berupa kebiasaan bekerja dengan beban berat, di mana kebiasaan bekerja dengan beban berat dapat menyebabkan pergesekan dari sendi lutut. Salah satu profesi yang banyak membutuhkan aktivitas fisik salah satunya adalah prajurit militer, di mana dari penelitian terdahulu didapatkan probabilitas terjadinya penyakit osteoarthritis pada prajurit militer mengalami peningkatan. TUJUAN: Diketahuinya pengaruh aktivitas fisik berlebih (active) serta faktorfaktor lainnya terhadap timbulnya penyakit osteoarthritis genu pada prajurit militer laki-laki di Jakarta. METODE: Untuk menentukan jumlah kasus osteoarthritis genu, dilakukan klasifikasi berdasarkan aktivitas fisik, olahraga, usia, genetik, lama bekerja, IMT, asupan makanan, beban berat, dengan menggunakan study case control. Analisis multivariat digunakan untuk mengetahui pengaruh yang paling signifikan, faktor risiko, dan interval kepercayaan 95% (IK 95% ) untuk osteoarthritis genu. HASIL: Sebanyak 51 kasus osteoarthritis genu didapatkan dari hasil kuesioner dan pemeriksaan VAS dengan jumlah populasi terjangkau (n=162). Ditentukan jumlah kasus 50 prajurit militer dan jumlah kontrol sebanyak 50 prajurit militer dengan matching 1:1 berdasarkan usia. 5 responden VAS (+) grade 4-5; 25 responden grade 5-6; 17 responden grade 6-7; 2 responden grade 7-8; 2 responden grade 8-9. Aktivitas fisik berlebih (active) memiliki memiliki pengaruh terhadap kejadian osteoarthritis genu (p = 0,023; OR = 15,55; 95% IK = 1,46–165,79). Faktor bekerja dengan beban berat juga memiliki pengaruh (p = 0,001; OR = 15,97; 95% IK = 3,31–77,02). Faktor riwayat keluarga dengan keluhan nyeri sendi juga memiliki pengaruh (p = 0,003; OR = 33,99; 95% IK = 3,43–337,29). Faktor asupan makanan tinggi kalsium dan vitamin D yang tidak rutin juga memiliki pengaruh (p = 0,025; OR = 3,77; 95% IK = 1.18–12,01). Terakhir IMT ≥ 26,85 juga memiliki pengaruh (p = 0,018; OR = 4,14 ; 95% IK = 0,02–4,14). KESIMPULAN: Pekerjaan sebagai prajurit militer dengan aktivitas fisik yang cukup tinggi, riwayat keluarga nyeri lutut, asupan makanan tinggi kalsium dan vit.D yang tidak rutin serta kebiasaan bekerja dengan beban berat terbukti memiliki pengaruh terhadap terjadinya osteoarthritis genu.

BACKGROUND : The arthritis presently is the highest cause of disability among retired military soldiers in the United States and is the main reason of the disabled soldiers. Arthritis occurs for many reasons. One of them is caused by activities that are often done. These various activities cause the joints to undergo continuous suppression which ultimately results in damage and inflammation. Physical activity can also be a habit of working with heavy loads, where heavy work habits can cause friction from the knee joint. One of the many professions require physical activity, one of them is a military soldier, where from previous studies, the probability of osteoarthritis disease in military soldiers has increased. OBJECTIVE : Knowing the effect of excessive physical activity (active) and other factors on the occurence of osteoarthritis genu disease in male military soldiers in Jakarta. METHODS: To determine the number of cases of osteoarthritis genu incidence. The classification based on physical activity, exercise, age, genetics, duration of work, BMI, food intake, heavy load and using a study control case were carried out. Multivariate analysis was used to determine the most significant influence, odds ratio, and 95% confidence intervals (95% CI) for osteoarthritis genu. RESULTS: A total of 51 cases of osteoarthritis incidence were obtained from questionnaires and VAS examination with an affordable population (n = 162). The number of cases of 50 military soldiers and the number of controls 50 military soldiers with the equivalent of 1: 1 based on age. 5 respondents VAS (+) grade 45; 25 respondents grade 5-6; 17 respondents grade 6-7; 2 respondents grade 7-8; 2 respondents grade 8-9. Physical activity (active) has a osteoarthritis genu risk of 15,55 times (p = 0.023; OR = 15,55; 95% CI = 1.46-165,79). The habits of working with heavy loads also have an effect (p = 0.001; OR = 15,97; 95% CI = 3,3–77,02). Family history factor with joint pain complaints also have an effect on the occurrence of osteoarthritis genu (p = 0,003; OR = 33,99 (95% CI = 3,4 – 337,29). Food intake factors of calcium and vitamin D are not routine also have an effect (p = 0.025; OR = 3,77 (95% CI = 1.18–12,01). BMI ≥26,85 also have an effect on the occurence of osteoarthritis genu (p = 0,018; OR = 4,14; 95% CI = 0.02–4,14) CONCLUSION: Occupations as military soldiers with high physical activity, family history of knee pain, high intake of calcium and non-routine vitamin D, heavy work habits, and BMI ≥ 26,85 have an influence and a probability of the risk of osteoarthritis genu.
