KORELASI PENGUKURAN ULTRASONOGRAFI INDEKS VENTRIKULAR INTRASEREBRI NEONATUS MENGGUNAKAN TRANSDUCER NONBAKU DENGAN TRANSDUCER BAKU DI RSUPN CIPTO MANGUNKUSUMO = Correlation between Ultrasonographic Measurement of The Neo natal Intracerebral Ventricular Index Using Non-standard and Standard Transducers in Cipto Mangunkusumo Hospital

Pendahuluan : Indeks ventrikular intraserebri (IV) neonatus adalah salah satu parameter yang digunakan untuk deteksi dini perubahan ukuran ventrikel. Ultrasonografi (USG) kepala merupakan pilihan pertama pada kasus ini. Ketersediaan transducer baku berupa transducer phased-array khusus bayi terbatas dibandingkan transducer nonbaku berupa transducer nonbaku yang berukuran lebih besar, berupa transducer curved-array dan transducer linear. Penelitian ini bertujuan untuk mengetahui korelasi hasil pengukuran USG IV neonatus menggunakan transducer nonbaku dengan transducer baku. Metode : Penelitian merupakan penelitian analitik dengan desain potong lintang pada neonatus yang dirawat di RSUPN Cipto Mangunkusumo (RSCM). Pemeriksaan menggunakan pesawat USG Medison Accuvix V20. Setiap neonatus dilakukan pemeriksaan USG kepala menggunakan transducer baku dan transducer nonbaku di saat yang sama. Gambar diambil masing-masing 3x untuk setiap jenis transducer dan pengukuran IV dikerjakan pada gambar yang tersimpan di pesawat USG pada waktu yang berbeda. Hasil : Rerata IV transducer baku 0,63 (0,38-2,31); rerata IV transducer nonbaku curved-array 0,72 (0,42-1,98); rerata IV transducer nonbaku linear-array 0,73 (0,431,84). Terdapat korelasi cukup kuat antara hasil pengukuran USG IV transducer nonbaku curved-array dengan transducer baku sehingga suatu formula regresi dapat dibentuk [IV transducer baku=0,19+(0,63 x IV transducer nonbaku curved-array)]; korelasi antara hasil pengukuran USG IV transducer nonbaku linear-array dengan transducer baku tidak cukup kuat. Kesimpulan : Terdapat korelasi yang cukup kuat antara hasil pengukuran USG IV menggunakan transducer nonbaku curved-array dengan transducer baku. Tidak terdapat korelasi yang cukup kuat antara hasil pengukuran USG IV transducer nonbaku linear-array dengan transducer baku tidak cukup kuat. Pengukuran IV menggunakan transducer nonbaku curved-array tidak dapat secara langsung menggantikan pemeriksaan menggunakan transducer baku, namun demikian nilai yang diperoleh dapat digunakan untuk mengestimasi nilai hasil pengukuran USG IV transducer baku melalui formula regresi [IV transducer baku = 0,19+(0,63 x IV transducer nonbaku curved-array)].

Introduction : Neonatal intracerebral ventricular index (VI) is one of parameters used for the early detection of changes in ventricular size. Head ultrasound (US) is the primary modality in this case. Availability of standard phased-array transducer is limited with small-foot print compared to non-standard transducer wiht bigger size which consisted of curved-array transducer and a linear transducer. This study aims to determine the correlation between US measurement of VI using non-standard and standard transducer and provides an alternative to measure VI using non-standard transducers when the standard transducer is not available. Methods : This is an analytic, cross-sectional study in neonates born at Cipto Mangunkusumo General Hospital. Using Medison Accuvix V20, each neonate had head US examination using standard and non-standard transducer at the same time. Images were taken 3x for each type of transducer and VI measurements were performed on the images stored in the US machine at different times. Results : The median VI for standard transducer is 0.63 ( 0,38 to 2,31 ); median VI for curved-array non-standard transducer is 0.72 (0,42 to 1,98) ; median VI for linear- array non-standard transducer is 0.73 (0,43 to 1,84). There is a fairly strong correlation between measurement of VI using curved-array non-standard transducer and the standard transducer; so that a regression formula can be established [standard transducer VI = 0.19 + (0.63 X VI curved-array non-standard transducer)]; correlation between VI measurements using linear-array non-standard transducer and the standard transducer is not strong enough. Conclusion : There is a fairly strong correlation between VI measurements using curved-array non-standard transducer and standard transducer. There is weak correlation between VI measurement using linear-array non-standard transducer and the standard stransducer. Neonatal intracerebral VI measurement using curved-array non-standard transducer cannot directly replace the measurement using the standard transducer, but it can be used to estimate the value of measurement using the standard transducer through a regression formula [standard transducer VI = 0.19 + (0.63 X VI curved-array nonstandard transducer)].
