Faktor-Faktor yang Mempengaruhi Angka Keberhasilan Portoenterostomi Kasai = Factors that influence the success rate of portoenterostomi Kasai.

Latar Belakang : Tatalaksana utama atresia bilier adalah pembedahan dengan tujuan utama mengembalikan aliran empedu dengan melakukan eksisi seluruh sisa duktus biliaris ekstrahepatik dan melakukan rekonstruksi dengan mengalirkan cairan empedu ke usus halus (yeyunum) melalui hubungan artifisial yang disebut portoenterostomi. Saat ini pasien atresia bilier di RSCM pada periode 2008 sampai 2013 belum pernah dilakukan evaluasi untuk menilai keberhasilan dari portoenterostomi Kasai. Tujuan : Penelitian ini dilaksanakan untuk mengetahui indikasi penderita atresia bilier yang dapat dilakukan operasi portoenterostomi Kasai. Metode : Penelitian ini adalah studi kohort retrospektif , yang menggunakan analisis regresi logistik. Sampel diambil dengan metode consecutive sampling. Pada subjek penelitian dilakukan pemeriksaan histopatologi untuk menilai derajat fibrosis dan sirosis dan laboratorium.. uji Chi Square digunakan pada variabel kategorik bila hasil tidak memenuhi syarat Chi Square maka dilanjutkan dengan uji Fisher Hasil : Prevalensi yang didapatkan dari 15 data angka keberhasilan berdasarkan kriteria bebas kuning 3 bulan sebesar 33,3% (5 dari 15 pasien) dengan angka kegagalan hingga 66,7%. Untuk melihat adanya kemungkinan hubungan antara derajat fibrosis dengan outcome hidup sampai dengan usia 1 tahun dilakukan perhitungan dengan uji Fisher didapatkan RR = 4 , nilai p = 0,04 dan 95% interval kepercayaan 1,5 – 10,65. Simpulan : Faktor - faktor yang dapat mempengaruhi keberhasilan tindakan kasai berupa bebas kuning 3 bulan pasca portoenterostomi Kasai pada pasien atresia bilier belum dapat diketahui. Terlihat kemungkinan derajat fibrosis mempengaruhi terhadap keberhasilan portoenterostomi Kasai untuk mencapai usia 1 tahun. Terdapat perbedaan karakteristik histopatologi dengan pasien atresia bilier di negara lain

Backgrounds : Management of biliary atresia is a major surgery with the primary objective to restore the flow of bile by excision entire extrahepatic bile duct and the rest of reconstructing the flow of bile into the small intestine (yeyunum) through artificial relationship called portoenterostomi. Currently in RSCM biliary atresia patients in the period 2008 to 2013 have not been evaluated to assess the success of the Kasai portoenterostomi. Aim : This study was conducted to determine the indication of biliary atresia patients do portoenterostomi Kasai operation. Methods : This was a cohort retrospectif study of consecutively-recruited billiary atresia patients. Review histopathology examination to classification degree of fibrosis and cirrosis . Further analysis using regresion logistic and Fisher’s test was performed. Results : Prevalence of data obtained from 15 success rate based on 3-month criterion yellow free of 33.3% (5 of 15 patients) with a failure rate of up to 66.7%. Correlation between the degree of fibrosis with life outcomes up to 1 year of age calculation by Fisher's exact test obtained RR = 4, p = 0.04 and 95% confidence interval 1.5 to 10.65. Conclusion : Factors that can affect the success of the action in the form of free kasai yellow portoenterostomi 3 months post-Kasai biliary atresia patients is not yet known. The possibility degree of fibrosis affecting the success of the Kasai portoenterostomi to reach the age of 1 year. There are differences in the histopathological characteristics of biliary atresia patients in other country.
