Penilaian Aktivitas Penyakit Lupus Eritematosus Sistemik dengan Skor SLEDAI di Departemen Ilmu Kesehatan Anak RSCM = Assessment of Disesase Activity in Pediatric Systemic Lupus Erythematosus with SLEDAI Scoring System at Department of Child Health Cipto Mangunkusomo Hospital.

Latar belakang. Penilaian aktivitas penyakit lupus eritematosus sistemik (LES) dengan skor Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) berperan penting dalam pemantauan atau follow up aktivitas penyakit LES pada anak. Saat ini belum ada data mengenai aktivitas penyakit LES anak dengan menggunakan skor SLEDAI setiap 3 bulan di Indonesia. Tujuan. Memantau aktivitas penyakit LES anak dengan skor SLEDAI setiap 3 bulan selama satu tahun pengamatan. Metode. Studi deskriptif untuk memantau aktivitas penyakit LES anak di Departemen Ilmu Kesehatan Anak Rumah Sakit Cipto Mangunkusumo secara retrospektif menggunakan data rekam medis dari bulan Juli 2005 hingga Juli 2013. Hasil penelitian. Jumlah subjek pada penelitian ini sebanyak 30 pasien. Mayoritas penderita LES adalah perempuan. Rerata usia awitan 11,23 (SD 2,88) tahun dan rerata usia saat diagnosis ditegakkan 11,79 (SD 2,69) tahun, terbanyak didiagnosis di atas usia 10 tahun dan tidak ada yang di bawah usia 5 tahun. Median (rentang) waktu antara timbulnya gejala sampai diagnosis ditegakkan adalah 3 (1–84) bulan dan terbanyak pada jarak kurang dari 5 bulan. Terapi inisial yang paling banyak diberikan adalah kortikosteroid dalam bentuk metilprednisolon. Manifestasi klinis awal tersering adalah artritis, rash, dan demam, sedangkan untuk laboratorium adalah peningkatan dsDNA dan komplemen darah yang rendah. Perbedaan skor SLEDAI terutama terlihat antara pengamatan bulan ke-0 dengan bulan ke-3. Skor SLEDAI yang dinilai setiap 3 bulan menunjukkan perubahan aktivitas penyakit LES yang bermakna, dengan mayoritas high activity pada awal pengamatan menjadi no activity pada akhir pengamatan. Simpulan. Penilaian skor SLEDAI setiap 3 bulan dapat digunakan untuk memantau aktivitas penyakit LES anak.

Background. Assessment of disease activity in pediatric systemic lupus erythematosus (SLE) with SLEDAI scoring system has an important role in monitoring or follow up disease activity of pediatric LES. Currently no available data that assess disease activity in pediatric SLE with SLEDAI scoring system every 3 months in Indonesia. Objective. To assess disease activity in pediatric SLE with SLEDAI scoring system every 3 months for one year observation. Methods. Descriptive study to assess disease activity of pediatric SLE at Department of Child Health Cipto Mangunkusumo Hospital using medical record retrospectively from July 2005 until July 2013. Results. Thirty patients were included in this study. Majority of SLE subjects were girls. Mean age at symptoms onset was 11.23 (SD 2.88) y.o and mean age at diagnosis was 11.79 (SD 2.69) y.o, most of them were diagnosed above 10 y.o and no one had below 5 y.o. The median of duration between symptoms onset and diagnosis was 3 (1–84) months, most of them had duration below 5 months. Majority of the subjects received corticosteroid in the form of methylprednisolone as initial therapy. Most common clinical manifestations were arthritis, rash, and fever, for laboratorium results were elevation of dsDNA and low complement level. The difference of SLEDAI score were especially obtained between the initial month with the 3 rd months. SLEDAI score that assessed every 3 months showed significant disease activity changes, with majority of patients had high activity in the beginnning and became no activity in the end of observation. Conclusions. Assesment of SLEDAI score every 3 months is useful for monitoring disease activity of pediatric SLE.
