Dressing for partial thickness burn using microbial cellulose and transparent film dressing: a comparative study at burn unit Cipto Mangunkusumo General Hospital, Jakarta Indonesia.

Introduction Burn is complex and devastating injuries. The healing process involves the regeneration of the epidermis and repair of the dermis. Pain is common and the wound exudation requires the change of dressing. Microbial cellulose, a polysaccharide synthesized in abundance by Acetobacter xylinum shows high potential in wound healing. It also creates a moist environment and the strong adhesion to the wound prevents bacterial contamination and protects from temperature changes. Transparent film dressing has been commonly used worldwide to cover partial thickness burn, because of its advantages such as easy to evaluate, rapid rate of epithelialization and provides high comfort for patients. Methods We perform a single blind randomized clinical trial study. All Patients with partial thickness burn with total body surface less than 20% area burns at any age presenting to our burn unit will be included in the study. They will be dressed using microbial cellulose and transparent film. Investigation forms are fill out and wounds will be evaluate on day 1, 3 ,7. And 10. The parameters are epithelization rate, patient’s level of pain, necessity of dressing change and cost effectiveness Results A total of 16 patient between 3 years old and 24 years old with an average of 15,7% body surface area partial-thickness burns were treated with microbial cellulose and transparent dressing. In a T test statistical analysis, patients treated with microbial cellulose, the epithelialization rate significantly faster than those treated with transparent dressing (microbial cellulose vs transparent dressing: 8,13 days vs 12,63 days with p value 0,000). In addition, the level of pain in patient using microbial cellulose is less than patient using transparent dressing,especially in day 3 and 10 (p=0,000). Related to cost effectiveness, in patient using microbial cellulose no need to change dressing, meanwhile in transparent dressing it need 2 to 3 times dressing changes, because of exudation. Conclusion The use of microbial cellulose have proven to show more effective compared to transparent dressing. The epithelization rate is faster ,the level of pain is less and the cost needed is cheaper than transparent dressing.
