Efek akut merokok kretek terhadap fungsi diastolik ventrikel kiri pada partisipan usia muda = Acute Effects of Clove Cigarette Smoking on Diastolic Function in Young Participants

Latar Belakang. Merokok merupakan faktor risiko yang paling mudah dimodifikasi dalam proses terjadinya gagal jantung. Di Indonesia, 88% konsumsi rokok adalah kretek. Sejauh ini belum ada studi mengenai hal ini terhadap fungsi ventrikel kiri. Penelitian ini akan melihat pengaruh akut rokok kretek pada partisipan usia muda dan membandingkan efek akut rokok kretek dengan rokok putih terhadap fungsi diastolik ventrikel kiri menggunakan ekokardiografi Metode. Uji eksperimental dilakukan di Departemen Kardiologi dan Kedokteran Vaskular Fakultas Kedokteran Universitas Indonesia/ Pusat Jantung Nasional Harapan Kita pada Maret - April 2013. Lima puluh partisipan dikategorikan sebagai bukan perokok rutin dan perokok rutin. Kesemuanya diminta tidak merokok minimal 2 jam sebelum penelitian. Pemeriksaan ekokardiografi dilakukan sebelum mulai merokok, segera setelah, dan satu jam setelah merokok rokok putih dan rokok kretek. Partisipan datang kembali di hari berikutnya untuk merokok jenis rokok lain. Hasil. Setelah merokok putih, nilai E/e’ septal kelompok bukan perokok rutin meningkat segera setelah merokok dan terus meningkat satu jam kemudian dibandingkan nilai dasar pengukuran, dengan rerata berturut-turut 7.63 + 1.63 dan 7.81 + 1.59, p = 0.000. Kelompok perokok rutin juga mengalami peningkatan rasio E/e’ septal segera setelah merokok hingga satu jam kemudian, rerata 7.76 + 1.31 dan 7.71 + 1.20, p = 0.000. Segera setelah merokok kretek, rasio E/e’ septal kelompok bukan perokok rutin meningkat hingga satu jam kemudian, rerata 7.53 + 1.58 dan 7.74 + 1.45, p = 0.000. Kelompok perokok rutin juga mengalami peningkatan E/e’ septal, rerata 7.74 + 1.45 dan 7.78 + 1.40, p = 0.000 Kesimpulan. Rokok menyebabkan perubahan akut fungsi diastolik ventrikel kiri pada kelompok perokok rutin dan bukan perokok rutin. Perubahan fungsi diastolik yang disebabkan rokok kretek berlangsung lebih lama pada kelompok bukan perokok rutin.

Background. Smoking is one of the most modifiable risk factor in heart failure. In Indonesia, 88% of cigararette smoked is clove cigarette. To the best of our knowledge, there were no studies published regarding this issue on left ventricular diastolic function. This sudy is to describe the acute effects of clove cigarette smoking on diastolic function in young participants and comparing the effects caused by clove cigararette to regular cigarette. Methods. This is an experimental study carried out in Department of Cardiology and Vascular Medicine Universitas Indonesia/ National Cardiavascular Center Harapan Kita in March - April 2013. Fifty participants divided into two groups: non daily smoker and daily smoker. Both groups were asked not to smoke for at least 2 hours prior to study. Echocardiography study was performed to before, right after and 60 minutes after smoking. Participants were then asked to come back on the next day to perform the same procedure with another kind of cigarette. Result. After regular cigarette smoking, there was an increased septal E/e’ from baseline in the non daily smoker group right after and 60 minutes after smoking, mean value of 7.63 + 1.63, 7.81 + 1.59 respectively, p = 0.000. In the daily smoker group, there was also an increase septal E/e profile, mean value of 7.76 + 1.31, 7.71 + 1.20), p = 0.000. After consumption of clove cigarette, a higher septal E/e’ was found in non daily smoker group, which lasting to 60 minutes after smoking, mean value of 7.53 + 1.58, 7.74 + 1.45), p = 0.000. Increased septal E/e’ was also showed in daily smoker group, mean value of 7.74 + 1.45, 7.78 + 1.40, p = 0.000 Conclusion. Clove and regular cigarette smoking have acute effects on left ventricular diastolic function in both non daily smokers and daily mokers. In comparison to regular cigarette, clove cigarette caused longer compromised diastolic function in non daily smokers.
