Ekstrak etanol daun mangifera foetida L. 0,25mg sebagai agen kelasi pada feritin serum penderita talasemia di RSCM tahun 2012 = Ethanol extract 0,25mg of mangifera foetida L. leaf as a chelator agent in serum feritin of thalassemia patient in RSCM 2012.

Talasemia merupakan penyakit herediter yang ditandai oleh penurunan sintesis rantai α atau β dari globin. Di Indonesia, prevalensi penderita talasemia mayor cukup tinggi. Menurut data RS Cipto Mangunkusumo Jakarta pada tahun 2008, terdapat 1412 pasien talasemia, yang mendapatkan terapi dengan mortalitas sebesar 0.8%. Pengobatan talasemia masih terbatas pada transfusi darah untuk menekan eritropoesis yang tidak efektif. Agen kelasi diperlukan untuk menurunkan akumulasi besi di organ akibat transfusi. Kelator yang biasa digunakan ialah desferoksamin, tetapi harganya mahal dan memiliki beragam efek samping. Oleh karena itu, diperlukan pengobatan yang aman dan terjangkau menggunakan ekstrak etanol Mangifera foetida L. yang mengandung mangiferin. Penelitian ini bertujuan untuk membuktikan bahwa ekstrak etanol daun Mangifera foetida L. 0.25 mg memiliki efek kelasi terhadap serum pasien talasemia. Desain penelitian berupa studi eksperimental pada 7 sampel serum pasien talasemia dibagi ke dalam 3 kelompok perlakuan secara exvivo. Parameter yang diukur adalah perhitungan nilai absorban tiap kelompok perlakuan (λ 200-500 nm). Uji normalitas Shapiro-Wilk menunjukkan data terdistribusi normal. Hasil penelitian dan perhitungan dengan uji statistik One Way Anova menunjukkan bahwa ekstrak etanol daun Mangifera foetida L. memiliki efek kelasi terhadap serum penderita talasemia (p=0,031). Uji Post Hoc menunjukkan bahwa ekstrak etanol dosis 0,25 mg memiliki efek kelasi yang hampir sama dengan kontrol positif mangiferin (p=0,138). Bila dibandingkan dengan ekstrak air, efek kelasi ekstrak etanol daun Mangifera foetida L. menunjukkan gambaran yang lebih baik. Hal ini diduga dipengaruhi oleh perbedaan kelarutan bahan aktif dalam etanol. Kata kunci: serum, talasemia, kelator, agen kelasi, mangiferin, Mangifera foetida L.

The prevalention of thalassemia cases in Indonesia is high enough. In Cipto Mangunkusumo Hospital, Jakarta, there are 1412 patients of thalassemia who get therapy with the mortality rate 0.8%, in 2008. Blood transfusion to suppress ineffective eritropoesis still becomes the choice of thalassemia therapy. Chelating agent is needed to decrease iron acumulation in body organs as the complication of transfusion. Chelator agent that is commonly used is Desferoxamin. However, it is expensive and having various side effects. Safe and affordable treatment of thalassemia is needed. This study aims to prove that the ethanol extract 0.25 mg of Mangifera foetida L. leaf has chelating effect to serum of thalassemia patient. Research designed which is used is experimental study with 7 serum as sample, divided to 3 groups by exvivo. Parameter measured is the calculation of absorban values in each group (λ 200-500 nm). Shapiro-Wilk test showed the data were normally distributed. The result of experiment using One Way Anova statistical test, showed that ethanol extract of Mangifera foetida L. leaf has the chelating effect on serum of thalassemia patients (p=0,031). According to the Post Hoc test, ethanol extract of 0.25 mg dose has chelating effect that nearly similar with mangiferin as a positive control (p=0.318). When compared with the water extract, ethanol extract of Mangifera foetida L. leaf showed the better result, that maybe influenced by the differences in solubility of the active substances in ethanol.
