Kadar acyl ghrelin plasma pada pendeita dispepsia dengan gambaran endoskopi mukosa lambung normal dan gastritis = Plasma acyl ghrelin concentrations in dyspeptic patients with normal gastric mucosa endoscopic findings and gastritis

Penelitian ini merupakan penelitian pendahuluan mengenai kadar acyl ghrelin plasma pada penderita dispepsia dengan gambaran endoskopi mukosa lambung normal dan gastritis di Jakarta. Acyl ghrelin plasma merupakan salah satu pemeriksaan baru sebagai petanda kondisi inflamasi dan atrofi mukosa lambung selain GastroPanel (Pepsinogen I, Pepsinogen II, perhitungan rasio Pepsinogen I/II, Gastrin dan antibodi H. pylori). Penelitian dilakukan di laboratorium Patologi klinik FKUI-RSCM bekerja sama dengan Divisi Gastroenterologi Departemen Ilmu Penyakit Dalam RSCM dan RS Ridwan Meureksa. Subyek penelitian terdiri dari 27 subyek dengan sindrom dispepsia dan telah dilakukan endoskopi dengan temuan gambaran endoskopi mukosa lambung normal (11.1 %) dan gastritis (88.9 %). Pemeriksaan kadar acyl ghrelin plasma menggunakan kit Elisa. Perbedaan kadar acyl ghrelin plasma berdasarkan jumlah lokasi gastritis dilakukan menggunakan uji t tidak berpasangan. Pada kelompok mukosa lambung normal didapatkan nilai rerata kadar acyl ghrelin plasma 23.59 pg/mL dengan rentang 21.98 – 24.56 pg/mL sedangkan pada kelompok gastritis didapatkan nilai median kadar acyl ghrelin plasma 11.35 pg/mL dengan rentang 0.36 – 15.59 pg/mL. Nilai rerata kadar acyl ghrelin plasma kelompok mukosa lambung normal lebih tinggi daripada kelompok gastritis. Didapatkan perbedaan bermakna kadar acyl ghrelin plasma antara 1 lokasi dengan lebih dari 1 lokasi dengan p = 0.029. Hasil penelitian ini mendapatkan kesan kadar acyl ghrelin plasma pada penderita dispepsia dengan gambaran endoskopi mukosa lambung normal tampak lebih tinggi daripada gastritis meskipun pada kelompok mukosa lambung normal jumlah subyek sedikit. Kadar acyl ghrelin plasma pada 1 lokasi lebih tinggi daripada lebih dari 1 lokasi.

This is a pilot study about plasma acyl ghrelin concentrations in dyspeptic patients with normal gastric mucosa endoscopic findings and dyspeptic patients with gastritis in Jakarta. Plasma acyl ghrelin is a new test use as a marker of inflammation and atrophic gastric mucosa beside the GastroPanel (pepsinogen I, pepsinogen II, pepsinogen I/II ratio, gastrin and H. pylori antibody). The study was conducted in the Laboratory of Clinical Pathology in cow orking with Gastroenterology Division of Department of Internal Medicine of Cipto Mangunkusumo National Hospital and Ridwan Meureksa Hospital. The study population consisted of 27 subjects with dyspepsia syndrome. Result of endoscopic findings showed normal gastric mucosa in 11.1% patients and gastritis in 88.9% patients. Acyl ghrelin concentration was measured using ELISA method. The difference of plasma acyl ghrelin concentration according to the number of gastritis location was statistically evaluated using unpaired t-test. In the normal gastric mucosa group, the mean concentration of plasma acyl ghrelin was 23.59 pg/mL with the range of 21.98 – 24.56 pg/ mL. In the gastritis group the mean concentration was 11.35 pg/ mL with 0.36 – 15.59 pg/ mL range. The plasma acyl ghrelin mean concentration was higher in the normal gastric mucosa group. We found a significant difference of acyl ghrelin plasma concentration between 1 lesion and more than 1 gastritis lesions (p = 0.029). The study showed that plasma acyl ghrelin concentrations of dyspeptic patients with the normal gastric mucosa was higher than gastritis although the number of normal subjects was limited. Plasma acyl ghrelin concentration was higher in gastritis with 1 lesion compared than more than one lesion.
