The effect of endometrial thickness in predicting the outcome of pregnancy among infertile patient who undergone in vitro fertilization (IVF) treatment

Objective: To study the effect of endometrial thickness during In Vitro Fertilization (IVF) cycles in predicting IVF outcome. Material and Methods: This retrospective study involved 306 infertile patients undergoing 336 IVF cycles in Yasmin Clinic, Dr.Cipto Mangunkusumo National Referral Hospital, Jakarta, Indonesia. All IVF cycles were performed from April 2005 until August 2009. The outcome of this study is the clinical pregnancy following IVF cycles. The endometrial thickness was measured on the day of human chorionic gonadotrophin (hCG) administration. Results: There were 77 among 336 cycles (22.9%) resulted in pregnancy. The endometrial thickness on day of hCG administration was significantly higher in pregnant group compared to non-pregnant group 11.30 (6.800 - 16.00) versus 10.00 (5.00 - 16.00); p < 0.0001). The result of multivariate analysis showed that apart from endometrial thickness, age of the patients and the number of embryos with ≥ 8 cells on cleavage II independently affects the outcome of IVF. Younger patient and increase number of embryo with ≥ 8 cells on cleavage had high tendency to get pregnant (adjusted OR: 0.937; P value: 0.032 and adjusted OR: 1.400; P value: < 0.0001 respectively). From the ROC curve, the endometrial thickness cut-off value ≥ 10.95 mm was the best value to predict pregnancy outcome. This value had 61.0% sensitivity, 59.6% specificity, 30.9% positive predictive value and 85.6% negative predictive value. Conclusion: Endometrial thickness, embryo quality and age of the patients are three independent factors which significantly have contribution to IVF outcome prediction. Thicker endometrium contributes to the better prognosis of IVF to achieve pregnancy.
