Comparison of admission blood pressure between ischemic and hemorrhagic stroke and the relation of blood pressure to stroke outcome.

Background and Problem - High admission blood pressure (Acute Hypertension) is a common manifestation in stroke patients. Despite that, differences of acute hypertension between ischemic and haemorrhagic stroke and relation to the outcome of patients have been understudied. This study aims to determine the differences of admission blood pressure between ischemic and haemorrhagic stroke and determine the relation of blood pressure to the outcome of patients in Indonesia. Methods- This is a cross-sectional study from year 2005 to 2009. Subjects were 747 stroke patients (haemorrhagic stroke = 298, Ischemic = 449) admitted into RSCM. Access to the registry for stroke patients was granted by the Neurological Department of RSCM. Outcome was assessed as either neurological recovery using the National Institute of Health Stroke Scale (NIHSS) or mortality. Mean blood pressure between both strokes types were compared using independent t-test; Pearson’s correlation test for determining association of blood pressure with neurological recovery and logistic regression for determining association with mortality. Results – The mean systolic, diastolic and mean arterial pressure for haemorrhagic stroke (179, 103 and 129 mm Hg) were significantly higher (p < 0.001) compared to ischemic stroke (164, 96 and119mm Hg). The correlation of blood pressure to neurological recovery showed no significance (Systolic; r = 0.038(p = 0.346), Diastolic; r = 0.03 (p = 0.457) and MAP; r = 0.037 (p = 0.363). The association of blood pressure to mortality, after adjustment, demonstrated no significance. [Systolic Ҳ2 : 1.53 (p = 0.215), 1.49 (p = 0.221), 1.54(p = 0.215)], [Diastolic Ҳ2 : 0.136 (p = 0.712); 0.338 (p = 0.561); 0.107 (p = 0.744) and [MAP Ҳ2 : 0.005 (p = 0.942); 0.001(p = 0.975) ; < 0.001(p = 0.998)] Conclusion- There is a higher admission blood pressure among hemorrhagic stroke patients. However, the admission blood pressure is not related with neurological recovery and mortality.
