An analysis of the influence of ovarian reserve on pregnancy in Indonesian women undergoing in vitro fertilization.
A recent review of population based surveys estimated that 72.4 million women out of the current world population are currently infertile, and of these, 40.5 million are seeking infertility medical care, of which in vitro fertilization (IVF) is immensely popular. This study is undertaken to investigate the role of widely accepted markers of ovarian reserve (AFC and basal FSH) in predicting clinical pregnancy outcome. The research was a retrospective cohort analysis of outcomes from IVF treatment. Based on inclusion and exclusion criteria, 305 patients undergoing 336 IVF-ET cycles in Klinik Yasmin RS Cipto Mangkunkusomo from April 2005 to November 2010 were analyzed and reviewed. 91.7% of pregnancies resulted in those with basal FSH levels < 10 IU compared to a mere 8.3% in those with abnormally high basal FSH levels. The ROC curve elucidated that at basal FSH levels < 6.74, women were 1.5 times more likely to become pregnant compared to women with higher FSH levels. The ROC curve also clearly demonstrated that women with 8 or more antral follicles were 2.485 times more likely to become pregnant than those with lesser folicles. It can be concluded that AFC and basal FSH do have clinical potential in predicting IVF outcome. The age of the patient should also be taken into consideration as it is significantly associated with both AFC and basal FSH.
