Outcomes in acute and negleted HIP fracture after hemiarthroplasty

Background: In developing countries, likes Indonesia. The availability of health care facilities and government policies may not sufficient and proper enough for patients to treat their acute hip fracture condition. All of these conditions above profound the rising of neglected hip fracture cases. Methods: Patients were retrieved from surgery outpatient clinic in PGI Cikini Hospital, Jakarta. Baseline data were obtained from medical records. Three researchers conducted the interviews using data collection sheet provided. Patients were classified according to their hip fracture onset type, acute or neglected. Participant’s ambulatory status was assessed by the time researchers doing their home visit along with functional outcomes assessment by Harris Hip Score. Result: From 17 patients that we retrieved through the medical record screening, 10 eligible patients were reviewed. The mean age was 75 years with, 80% of participants were females, mean BMI score recorded was 23, 70% of patients were categorized into acute cases. The mean Harris hip score recorded was 63 (SD 28). The Independent samples Kolmogorov-Smirnov test for hip fracture case and HHS class variable showed no significant association (p-value = 0.835). The Chi-square test showed significant association between ambulatory status and HHS class (p-value = 0.036). The Chi square test showed no significant association between type of hip fracture case and ambulatory status (p-value = 0.260). Conclusion: The female gender, older age (above > 75 years old), and femoral neck fracture were the risk factors of the hip fracture occurrences, therefore putting patients as the candidates of hemiarthroplasty procedure. There was not any association found between the types of hip fracture case, acute or neglected with functional outcomes as assessed by Harris hip score and ambulatory status following hemairthroplasty procedure.

Latar Belakang: Di negara-negara berkembang, seperti Indonesia. Ketersediaan fasilitas pelayanan kesehatan dan kebijakan pemerintah mungkin tidak cukup dan cukup layak untuk pasien untuk mengobati pinggul kondisi fraktur akut mereka. Semua kondisi di atas mendalam meningkatnya kasus patah tulang pinggul diabaikan. Metode: Pasien yang diambil dari klinik bedah rawat jalan di Rumah Sakit PGI Cikini, Jakarta. Baseline Data diperoleh dari catatan medis. Tiga peneliti melakukan wawancara dengan menggunakan lembar pengumpulan data yang disediakan. Pasien diklasifikasikan menurut jenis mereka fraktur hip onset, akut atau diabaikan. Peserta status rawat jalan itu dinilai oleh para peneliti waktu melakukan kunjungan rumah mereka bersama dengan hasil penilaian fungsional oleh Skor Hip Harris. Hasil: Dari 17 pasien yang kami diambil melalui pemutaran rekam medis, 10 pasien yang memenuhi syarat telah dibahas. Usia rata-rata adalah 75 tahun dengan, 80% dari peserta adalah perempuan, berarti BMI skor dicatat adalah 23, 70% dari pasien dikategorikan ke dalam kasus akut. Harris berarti hip skor yang tercatat adalah 63 (SD 28). Sampel Kolmogorov-Smirnov Independen uji untuk kasus patah tulang pinggul dan variabel HHS kelas tidak menunjukkan hubungan yang signifikan (p-value = 0,835). Uji Chi-square menunjukkan hubungan yang signifikan antara status rawat jalan dan kelas HHS (p-value = 0.036). Uji Chi square menunjukkan ada hubungan yang signifikan antara tipe kasus fraktur pinggul dan status rawat jalan (p-value = 0,260). Kesimpulan: jenis kelamin perempuan, usia tua (diatas > 75 tahun), dan fraktur leher femur merupakan faktor risiko dari kejadian patah tulang pinggul, sehingga menempatkan pasien sebagai calon prosedur hemiarthroplasty. Tidak ada hubungan yang ditemukan antara jenis kasus patah tulang pinggul, akut atau diabaikan dengan hasil fungsional yang dinilai dengan skor hip Harris dan status rawat jalan mengikuti prosedur hemairthroplasty.
